About this file
This document is a Responsibility Inquiry form for Solicitation S-66000-00013715 issued by the State of Oregon for the Oregon SeaSketch Tool Maintenance and Technical Development project. The form is designed to assess an offeror's capability, financial stability, legal standing, and capacity to perform the contract by requesting detailed information about the company's past performance, financial health, legal history, and operational readiness.
The inquiry covers multiple critical areas of vendor assessment, including the offeror's ability to complete similar contracts within time and budget, any criminal or civil charges related to public contracts, contract terminations, financial distress, bankruptcy history, licensing, insurance, and pay equity certification. Specifically for contracts employing 50 or more full-time workers with a contract value exceeding $500,000, vendors must possess a current Pay Equity Certificate from the Department of Administrative Services. The form requires comprehensive self-reporting and allows the agency to comprehensively evaluate a vendor's qualifications and reliability before contract award.
