About this file
This document is an Offeror Information and Certification Sheet for Solicitation S-66000-00013715 issued by the State of Oregon for the Sea Sketch Tool Maintenance and Technical Development project. The certification form requires potential vendors to provide key company information and affirm compliance with various state regulations, non-discrimination policies, and legal requirements. While specific details about the Sea Sketch Tool project are not extensively elaborated in this document, the form indicates that the contract opportunity involves technical development and maintenance services, with a firm offer period of 180 days and potential for contract execution.
The certification sheet emphasizes multiple compliance requirements for potential contractors, including tax law adherence, non-discrimination policies, workforce equity standards, and conflict of interest disclosures. Notable requirements include having a written policy preventing sexual harassment, complying with wage equity laws, and ensuring the vendor is not on federal exclusion lists. The document suggests a potential contract value of at least $150,000, with strict guidelines around employee treatment, compensation transparency, and alignment with Oregon state procurement regulations. Vendors must also certify the independence and truthfulness of their proposal, with potential legal consequences for false claims or misrepresentations.
