About this file
This is a Reply Format Attachment for ITN No. DMS-26/27-025, issued by the Florida Department of Management Services (DMS) for the MyFloridaNetwork-3 project. The document provides standardized instructions for respondents to prepare and submit proposals in response to this invitation to negotiate. The ITN seeks a contractor to provide network services, with responses required to follow a specific nine-tab organizational structure including introduction, mandatory certifications, cost reply, qualifications and experience, client references, staffing plan and key personnel, technical reply, other required forms and documentation, and contract redlines. Respondents must submit their replies according to the specified format with all documents organized as separate electronic files with designated naming conventions. The document does not specify explicit response deadlines, site visit requirements, or bidders' conferences within this attachment, though it references other sections of the ITN for submission procedures.
The cost reply must be submitted separately in Microsoft Excel format under TAB 3 using Attachment K, the Cost Reply Price Workbook, with detailed instructions provided in Attachment G. Respondents must demonstrate financial capability by submitting a letter of credit dated on or after June 1, 2025, from a Florida-authorized surety company documenting the ability to obtain a performance bond of at least sixty million dollars ($60,000,000); failure to provide this letter will render the reply nonresponsive. Additional financial requirements include audited financial statements for the last three years with the most recent audit not more than one fiscal year and 120 calendar days old from the solicitation advertisement date, containing a signed audit opinion from a CPA without adverse or disclaimer opinions. Respondents must also obtain a Service Provider Identification Number (SPIN) from the Universal Service Administrative Company (USAC) and submit current Service Provider Annual Certification (SPAC) forms to demonstrate E-rate program eligibility, as DMS seeks to obtain E-rate funding for all eligible services under the contract.

Introduction The Department seeks to make Reply preparation easy and efficient, while giving Respondents ample opportunity to highlight their Replies. The evaluation and negotiation processes must also be manageable and effective.
The Respondent’s Reply should demonstrate an understanding of, and the ability to, meet and perform all contractual requirements listed in this ITN, including all contractual services.
This Proposal Format describes the contents and format designed to encourage completeness in the Respondent's Reply. The State’s goal is to standardize the format of the Replies to allow comparisons and facilitate equitable measurements for evaluation.
Reply Preparation Costs Any and all costs incurred by the Respondent in preparing or submitting a Reply shall be the Respondent’s sole responsibility whether or not any award results from this ITN. The State shall not reimburse any costs.

The contents of each response submittal shall be organized as listed in the Table provided below with the exception of the Cost Reply. The Cost Reply is to be provided separately under “TAB 3” as described in the table below and labeled as “Cost Reply.” No pricing or cost information is to be included in any portion of the Respondent’s Reply other than TAB 3. Failure to comply with this requirement will result in disqualification.
If proprietary or confidential information is included in the Reply, the Respondent shall follow instructions provided in Section 3.9(“Public Records, Respondent’s Confidential Information, and Redacted Replies”) of this ITN in submitting these files. Respondent shall include a descriptor in the file name to designate that the file represents proprietary/confidential information. If applicable, one (1) REDACTED electronic copy of the entire Reply should be submitted. If a redacted Cost Reply is submitted, it must be submitted as a separate digital f ile.

In completing Replies, Respondents should emphasize the quality, completeness, and clarity of content. Respondents are limited to submitting one Reply to this ITN. Modifications or withdrawal of Replies must be in accordance with Sections 2.4 and 2.5 of the ITN.
