About this file
This is a Sample Work Order Proposal Request (WOPR) form for the County of Santa Clara Health System Behavioral Health Services Department, issued under RFSQ-BHS-FY25-0176 for Behavioral Health Training Services. The document serves as a template for vendors to submit proposals for Emotional Well-Being services, specifically health coaching and education. Vendors are required to provide descriptions of services including training objectives, outlines, presentations, and materials; demonstrate relevant work experience with two examples from the past three years; and identify qualified practitioners, coaches, or instructors with relevant education, credentials, certifications, and years of experience in the requested service areas.
The pricing structure allows vendors to propose rates at or below the County's contracted rates across multiple service categories including wellness coaching (ranging from 30-minute to full-day sessions), health education and promotion seminars (under 2-hour to 8-hour sessions), well-being training or certifications, manager and supervisor support, and online resource libraries with enterprise licensing options. The County will not reimburse vendors for travel, travel-related expenses, or out-of-pocket costs. Pricing proposals are submitted by service category and cost unit, with designations for onsite or virtual delivery. All vendor responses must comply with the terms and conditions of the master Agreement for Services and Deliverables (referenced as Exhibit C), and in cases of conflict, the Agreement shall control. The County is subject to California Public Records Act disclosure requirements, and vendors must clearly mark any proprietary information as "CONFIDENTIAL AND PROPRIETARY" with specific line identification.
