About this file
This is Addendum #1 to a Request for Statement of Qualifications (RFSQ) issued by the County of Santa Clara, California Behavioral Health Services Department (BHSD) for Behavioral Health Training Services. The addendum, issued on January 28, 2025, by Contracts Manager Angeleah Macatiag, incorporates several revisions to the original RFSQ-BHS-FY25-0176. Key changes include the removal of "Seeking Safety Training" from Category 1: Evidence-Based Practice Trainings across all applicable documents (the Cover Sheet, Evaluation Criteria and Qualifications, and Scope of Work), updates to the Executive Summary requirements to clarify that it must not exceed one page and is separate from the maximum allowable page count, and restructuring of Section IV.E.1 to remove Appendix B from Table 4 with subsequent renumbering. The RFSQ seeks qualified firms to provide health and professional training services supporting County clinical staff and County-contracted Provider staff. Procedural questions may be submitted through the Periscope portal by Friday, March 1, 2030, at 3:00 P.M. Pacific Time.
The addendum clarifies several submission and evaluation requirements through responses to frequently asked questions from the January 21, 2025 Offerors' Conference. Offerors representing institutions must select all applicable experience types and confirm their organization meets minimum qualifications outlined in the Cover Sheet. Syllabi may be submitted as miscellaneous supporting documents and must be referenced in the response narrative. The Insurance Exhibit and Endorsement Letter are required at the time of offer submission. Appendix B–Evaluation Criteria and Qualifications is required only as part of Section IV.E.2 in the Response Submission, not as a separate attachment. The RFSQ operates on a two-phase process: Phase One establishes a qualified pool of providers, while Phase Two involves a Work Order Proposal Request (WOPR) process to address contract values and specific service requirements as projects are identified. All training formats (in-person, hybrid, virtual) are encouraged, though format may be specified during Phase Two based on identified training needs. Current BHSD County-contracted providers must apply through this RFSQ process to remain eligible for future training opportunities.

If the Offeror is an institution rather than an individual, should it check off each type of experience its employees have, or simply check the box to confirm that it meets above qualifications outlined in Appendix A - Cover Sheet, Section I.1?
Offeror should select all that apply AND then confirm that Offeror’s organization meets the minimum qualifications outlined in Appendix A - Cover Sheet, Section I.1.

May an Offeror propose to a training that is not yet fully designed but for which the Offeror is qualified to teach, as referenced in Appendix A - Cover Sheet, Section I.2? Or must all trainings be fully developed at the time of application?

4. Q:
If the Offeror is a Care Facility/SUTS Provider as well as a Healthcare Facility Provider, but these other two units are not involved with the proposed program, is it still necessary to provide licensing details in Appendix K- Offeror’s FEIN and License Information Form?
No, it is not necessary to provide licensing details if they are not a part of the training service.
