About this file
This is a Cyber Supply Chain Risk Management (C-SCRM) Plan Questionnaire template that is part of RFP #80GSFC27R0015, issued by NASA Goddard Space Flight Center in August 2026 for an unspecified contract. The questionnaire serves as an optional alternative to submitting a full C-SCRM plan compliant with NIST SP 800-161, with contractors permitted to submit either document but not both.
The questionnaire comprises six substantive sections designed to establish a baseline understanding of contractor cybersecurity practices. Section 1 collects corporate information including company name, contract details, and primary points of contact. Section 2 addresses supply chain provenance, requiring verification that suppliers of covered articles are identified, ownership is confirmed, critical suppliers remain under U.S. ownership, and Basic Security Requirements from NIST SP 800-171 are implemented. Section 3 focuses on supply chain management and supplier governance, requiring documentation of threat identification, supplier mapping, contractual SCRM requirements, and verification processes. Section 4 addresses physical and personnel security, including background check policies and insider threat training. Section 5 covers supply chain integrity and resilience through lifecycle management, end-of-life requirements, and documented resilience processes. Section 6 requires detailed hardware and software inventories with specific data fields including original equipment manufacturer information, model and serial numbers, country of origin, NDAA Section 889 compliance status (for telecommunications and video surveillance equipment), and physical location. The questionnaire applies to all contractor information systems and components regardless of FIPS 199 impact level, with mandatory explanation required for any "No" responses. Questions should be directed to agency-dl-ocio-cys-cp-scrm@mail.nasa.gov and agency-dl-ocio-rca@mail.nasa.gov.

ATTACHMENT NASA C-SCRM PLAN QUESTIONNAIRE
AUGUST 2026
CONTRACT TBD
The DRD, "Security Requirements for Unclassified Information Technology Resources," requires a Cyber Supply Chain Risk Management (C- SCRM) Plan consistent with the recommended template in NIST SP 800-161. This document may be found at: This document may be found at: https://csrc.nist.gov/pubs/sp/800/161/r1/upd1/final. Appendix A provides C- SCRM security controls, Appendix B a C-SCRM control summary and Appendix D C-SCRM Templates.
This questionnaire aligns with the above referenced NIST template and is intended to make fulfilling this requirement easier. At this time, the Office of the Chief Information Officer (OCIO) questionnaire is optional. The OCIO will accept either a plan that complies with NIST SP 800- 161 or this questionnaire. Contractors DO NOT need to submit both a plan and this questionnaire. If a contractor has an existing C-SCRM plan that complies with NIST SP 800-161, there is no requirement for the contractor to complete the OCIO questionnaire.
For questions, please email agency-dl-ocio-cys-cp-scrm@mail.nasa.gov and Cc agency-dl-ocio-rca@mail.nasa.gov.
