The contract will be awarded under FAR Part 12 for commercial items. All contractors must be registered in the System for Award Management to be eligible for award.

This questionnaire is designed to provide Defense Logistics Agency (DCSO-F) with basic information regarding the Contractor’s past performance on previous contracts they have been awarded and the satisfaction with their performance.
