About this file

This file is a Vehicle Mileage Log (VML) form template for the Florida Department of Children and Families (DCF), specifically designed for travel reimbursement documentation by employees such as Jessi Dyer. The form serves as a standardized mechanism for recording individual employee travel details, including departure and arrival locations, times, odometer readings, and mileage claims. The template accommodates up to 97 line entries across five pages for documenting multiple trips and includes fields for map mileage (based on FDOT Official Highway Mileage Viewer), vicinity mileage (miles driven over what FDOT permits), and incidental expenses such as tolls. The form requires employees to provide nine-digit PeopleFirst (PF) identification numbers, full office and home addresses, trip purposes without acronyms, and precise odometer readings to substantiate claims. Electronic signature attestation is required, confirming the accuracy of submitted data and legal binding nature of the submission.

The VML form implements DCF's shortest distance rule as a benefit to the state, which requires employees to claim the shorter of either actual home-to-worksite mileage or headquarters-to-worksite mileage, with three notation options indicating travel circumstances: asterisk notation for home as shortest distance, double asterisk for headquarters as shortest distance benefit, and triple asterisk for travel occurring one or more hours outside standard 8:00 AM to 5:00 PM Monday-Friday operating hours. Odometer mileage must align with map and vicinity mileage totals unless the shortest distance rule applies, and employees are explicitly prohibited from altering actual points of origin, destinations, or odometer readings—only claimed mileage may be adjusted. The form references integration with the State Travel Management System (STMS) and the FDOT Mileage Viewer for verification purposes, establishing it as a supporting document for the broader Travel Mileage Reimbursement Application that DCF is procuring through ITB 2425 077.

*Travel to/from home is a benefit to the state* to be used in the Shortest Distance Notation column when traveler is traveling directly to/from home to indicate it is the shorter distances and they are permitted to claim the full mileage supported by their odometer reading
**Not Actual Mileage. The claim is to/from HQ as it is a benefit to the State** to be used in the Shortest Distance Notation column when traveler is traveling directly to/from home to indicate HQ is the shorter distance and they are claiming less miles than what is supported by their odometer reading because the claim is between HQ and worksite as a benefit to the state
***1+ hour before/after normally scheduled work hours*** to be used in the Shortest Distance Notation column when traveler is traveling directly to/from home to indiciate travel is taking placing 1+ hours before/after normally scheduled work hours to indicate they are permitted to claim the full mileage supported by their odometer reading
NOTE: Odometer mileage must agree with the total of the map/vicinity mileage unless shortest distance rule is applied as a benefit to the state.

Shortest Distance Rule:

If a traveler is physically leaving/returning from their home address, they must claim the mileage from their HQ address to the worksite if that is the shortest mileage distance, as a benefit to the state. The physical location of the departure/return must be notated on the mileage log as well as the accurate odometer readings for it to be a true representation of the trip; however, the mileage claimed must be the shortest distance possible as a benefit to the state, regardless of working from home. At no point should a traveler alter their actual point of origin, point of destination or their odometer readings. The only thing that can be adjusted is the mileage being claimed.

The mileage log would then have notations, as outlined such (legend provided at the bottom of Page 1 of the Mileage Log):

*Travel to/from home is a benefit to the state (this means you are attesting that your home address was indeed the shortest distance) **Not Actual Mileage. The claim is to/from HQ as it is a benefit to the State (this means that the physical address you departed/returned is listed, but you are claiming the shortest mileage from the HQ address to the worksite as a benefit to the state) ***1+ hour before/after normally scheduled work hours (the only exception to this rule is if the traveler left 1+ hours before or returned 1+ hours after normally scheduled work hours). NOTE: DCF peak hours of operation are 8:00am – 5:00pm Monday – Friday. If the traveler has an approved, alternate schedule, that must be notated if claiming this exception.

EXAMPLE:

-Traveler leaves home and goes to their work site address and the mileage is 40 based on the odometer readings -HQ address to work site address is 60 miles -The mileage claimed can then be the 40 miles to/from their home address based on the shortest distance rule and the above * notation would be put in the "Shortest Distance Notation" column on the mileage log to indicate that the mileage being claimed to/from their home address is the shortest distance as a benefit to the state.

-Traveler leaves home and goes to their work site address and the mileage is 60 based on the odometer readings -HQ address to work site address is only 40 miles -The mileage claimed can only be 40 based on the shortest distance rule and the above ** notation would be put in the "Shortest Distance Notation" column on the mileage log to indicate that the mileage being claimed is less than what the odometer readings shows because they are claiming the shortest distance as a benefit to the state.

-Traveler leaves prior to 7am or arrives back to their home after 6pm. They can then claim the mileage traveled to/from their home regardless of shortest distance. Then, the above notation *** would be put in the "Shortest Distance Notation" column on the mileage log to indicate that the travel took place 1hr+ before/after employee’s normally scheduled work hours.
