About this file

This is an Offeror Information and Certification Sheet for RFP #S-44300-00010099 issued by the State of Oregon Department of Administrative Services (DAS) Procurement Services for a Vista New Member Orientation Location for Meetings. The document serves as a certification form that requires offerors to provide their legal business name, address, contact information, state of incorporation, entity type, and Oregon Secretary of State Business Registry Number. Offerors must certify their understanding and acceptance of RFP requirements, acknowledgment of all addenda, compliance with performance standards, and absence of undisclosed liquidated debt owed to the State. The document establishes the baseline administrative and legal requirements for bid submission without specifying product deliverables, quantities, response deadlines, or contract award dates.

The certification form requires offerors to attest to compliance with Oregon tax laws, non-discrimination in employment practices, adherence to sexual harassment prevention policies for contracts valued at $150,000 or more, and verification that neither the offeror nor its employees, agents, or subcontractors appear on federal exclusion lists maintained by the Office of Foreign Assets Control or the System for Award Management. Offerors must also certify the absence of conflicts of interest, truthfulness and accuracy of bid contents, and independent preparation without collusion. The document references potential liability under the Oregon False Claims Act for false or fraudulent statements. No pricing information, set-asides, incumbents, or funding sources are addressed in this certification sheet, as it functions solely as an attestation document requiring authorized representative signature and date.
