SMF Pedestrian Walkway Hub - Flight Paths Public Art Installation Attachment A Attachment A

Sacramento County Department of Airports SMF Pedestrian Walkway Hub - Flight Paths Public Art Installation Project

ATTACHMENT A

MINIMUM CONTRACTOR QUALIFICATIONS

CUPCCAA Informal Bid | Contract No. 41015 | September 2026 The following minimum qualifications are required for the Bid to be considered responsive and for the Bidder to be determined responsible. The Bidder shall submit a Contractor Qualification Statement, Self-Performance Certification, and all supporting information required by this Attachment with its Bid.

The County may reject a Bid as nonresponsive if the Bidder fails to submit material qualification information required by this Attachment. The County may determine a Bidder to be non-responsible if the information submitted does not demonstrate compliance with the minimum qualifications.

The County may request clarification or verification of information timely submitted with the Bid. Clarification may explain or verify submitted information but shall not be used to:

• Cure the failure to submit a material qualification;

• Add a construction subcontractor;

• Substitute another entity’s experience for the Bidder’s required experience;

• Replace proposed key personnel in a manner that materially changes the Bidder’s qualifications; or

• Materially revise the Bid after bid opening.

1. LICENSE AND REGISTRATION

The Bidder shall hold, at the time required by California law and throughout performance of the Work, an active and valid California contractor license in the following classification:

• C-61/D-34 – Prefabricated Equipment Contractor.

A Class B — General Building Contractor license, without the required C-61/D-34 classification, does not satisfy this requirement.

The required license shall be legally sufficient for all Work assigned to the Contractor under the Contract Documents. Identification of the C-61/D-34 classification in the Notice Inviting Informal Bids, OpenGov posting, Bid Documents, Contract Documents, or this Attachment does not authorize the Contractor to perform work outside the scope permitted by California law.

The Contractor-assigned Work includes receiving, inspecting, inventorying, staging, protecting, assembling, rigging, lifting, positioning, connecting, aligning, adjusting, and installing County-furnished prefabricated suspended artwork components and associated artwork-installation hardware.

Unless expressly added by written Addendum before bid opening or by written Change Order after award, and legally authorized under the Contractor’s license, the Contractor’s Work does not include:

• Fabrication of the permanent artwork;

• Installation, modification, or repair of the permanent suspension cables;

• Installation, modification, or repair of the primary structural attachment infrastructure;

• Permanent structural steel fabrication or erection;

• Permanent structural welding;

• Structural reinforcement or permanent building modification;

• Electrical work;

• Glazing or curtain-wall modification other than the expressly detailed six bird attachments and approved Flight Path A, Sections 2 and 4 connection locations; or

• Permanent-design engineering for the artwork, permanent suspension system, or structural support system.

The Bidder shall provide:

• Contractor legal name;

• Contractor license number;

• C-61/D-34 classification;

• License expiration date;

• Department of Industrial Relations public works contractor registration number; and

• Certification that the required license and registration will be maintained throughout performance of the Work.

The County reserves the right to verify the Bidder’s license and registration status with the Contractors State License Board and Department of Industrial Relations.

2. SELF-PERFORMANCE

The Contractor shall self-perform one hundred percent (100%) of all field construction and artwork-installation Work using its own employees.

Construction subcontractors and lower-tier construction subcontractors are not permitted. The Bidder shall identify “None” on the County’s designation-of-subcontractors form and any other subcontractor-listing form included with the Bid.

For purposes of this requirement, the Contractor’s own employees are workers who are directly employed and paid by the Contractor and are under the Contractor’s direct supervision and control.

The following entities are not considered construction subcontractors, provided they do not perform field construction or artwork-installation labor:

Material and equipment suppliers;

Common carriers and delivery companies;

Equipment-rental companies;

Manufacturer representatives providing technical information, product support, or training;

Licensed professional engineers providing calculations or professional services for temporary works or Contractor means and methods; and Independent testing or inspection agencies.

The Contractor remains fully responsible for all products, equipment, services, calculations, information, and assistance furnished by such entities.

Except when Attachment D expressly states that the County will provide an operator, personnel operating lift, access, or installation equipment and personnel performing rigging or artwork-installation Work shall be employees of the Contractor.

The Bidder shall submit a signed Self-Performance Certification confirming that:

• All field construction and artwork-installation Work will be performed by the Bidder’s own employees;

• No construction subcontractors or lower-tier construction subcontractors will be used;

• The Bidder has sufficient direct workforce and supervision to complete the Work within the required Contract Time;

• The Bidder will not subcontract any field construction or artwork-installation Work after award; and

• The Bidder will comply with the self-performance requirements throughout performance of the Contract.

The self-performance requirement is a material requirement of the Contract. Unauthorized subcontracting may constitute grounds for rejection of the Bid, withholding of payment, removal of unauthorized personnel, termination, or other remedies available under the Contract Documents and applicable law.

3. COMPARABLE PROJECT EXPERIENCE

The Contractor shall have successfully completed at least three (3) comparable projects within the last five (5) years involving suspended public art, museum/exhibit installation, architectural feature installation, rigging-intensive installation, or comparable specialty installation work.

At least one (1) comparable project shall have included suspended or overhead installation above or adjacent to finished public, occupied, or sensitive facility areas.

At least one (1) comparable project shall have been performed in an airport, transportation facility, museum, civic/public facility, active commercial facility, or similarly sensitive finished environment.

The Bidder shall have directly performed the work used to satisfy this requirement. Experience belonging solely to a subcontractor, supplier, consultant, affiliate, parent company, sister company, or other separate legal entity shall not satisfy the Bidder’s comparable-project experience requirement.

For each comparable project, the Bidder shall provide:

• Project name and location;

• Owner or client;

• Description of the completed work;

• Bidder’s directly performed scope;

• Contract value;

• Completion date;

• Owner or client contact name, telephone number, and email address; and

• Explanation of how the project is comparable to this Project.

The County may contact the listed references and verify the information provided.

4. SPECIALTY ARTWORK-INSTALLATION EXPERIENCE

The Bidder shall demonstrate direct experience performing the following activities for artwork, exhibits, architectural features, or comparable prefabricated specialty components:

• Receiving and inspecting delivered components;

• Preparing component inventories and condition documentation;

• Staging, handling, and protecting sensitive components;

• Assembling prefabricated sections;

• Planning and performing rigging and lifting operations;

• Positioning and connecting suspended components;

• Performing final alignment and adjustment;

• Protecting artwork, finished facilities, and adjacent construction; and

• Completing punch-list, documentation, certification, warranty, and closeout requirements.

The Bidder shall identify the comparable projects through which it obtained this experience.

5. RIGGING, LIFT, AND EQUIPMENT QUALIFICATIONS

The Bidder shall demonstrate that its proposed personnel possess training, certifications, licenses, or documented qualifications appropriate for:

• Rigging and suspended-load control;

• Elevated work;

• Operation of the proposed lift or access equipment;

• Signal-person and spotter responsibilities;

• Fall protection;

• Equipment inspection; and

• The proposed artwork-installation and access methods.

When County-furnished lift or access equipment will be used, the Bidder shall demonstrate that its proposed employees are qualified to inspect, operate, and safely use the equipment unless Attachment D expressly states that the County will provide an operator.

When the final County-furnished equipment has not been identified at the time of Bid, equipment-specific operator documentation may be submitted with the Rigging, Lift, Equipment, and Qualification Package required by Item 4 of Attachment B. The Bidder shall nevertheless demonstrate with its Bid that it has the direct workforce and organizational capacity to provide appropriately qualified personnel.

The Bidder shall not rely on the qualifications of a proposed equipment-rental-company operator, construction subcontractor, or other separate legal entity to satisfy a qualification assigned to the Bidder’s field employees.

6. FINISHED FACILITY PROTECTION EXPERIENCE

The Bidder shall demonstrate experience protecting finished and sensitive facilities during specialty installation work.

Applicable experience may include protection of:

• Finished flooring and terrazzo;

• Glazing and curtain walls;

• Walls, ceilings, doors, and architectural finishes;

• Thresholds and expansion joints;

• Public circulation areas;

• Occupied or near-occupied areas; and

• Active airport, transportation, museum, civic, or commercial facilities.

The Bidder shall identify the comparable projects through which it obtained this experience and briefly describe the protection methods used.

7. SAFETY QUALIFICATIONS

The Bidder shall disclose its safety record and demonstrate its ability to provide Project-specific safety documentation, including IIPP, CSWP, CSSP, task-specific safety planning for high-risk activities, lift/rigging safety planning, fall-protection planning, emergency-response procedures, and controlled-access-zone procedures.

8. PROJECT COORDINATION

The Contractor shall demonstrate ability to coordinate with the County, Department of Airports, Airport Operations, Airport Security, Project Inspector, Artist or Artist’s representative, Engineer/SEOR when required, CMID, commissioning teams, adjacent contractors, and other project stakeholders.

9. BONDS AND INSURANCE

The Contractor shall demonstrate ability to provide all required bonds, insurance, endorsements, and other contract compliance documents required by the Contract Documents.

10. REQUIRED QUALIFICATION SUBMITTAL

The Bidder shall submit the Contractor Qualification Statement and all information listed below with its Bid. The three-Working-Day post-bid timing in Section 2-6 of the Standard Construction Specifications does not apply to this Project-specific qualification submittal:

• Completed Contractor Qualification Statement;

• Contractor legal name and contact information;

• Contractor license number and C-61/D-34 classification;

• Department of Industrial Relations registration number;

• Signed Self-Performance Certification;

• County designation-of-subcontractors form identifying “None” for construction subcontractors;

• Three comparable project references, including project name, owner, location, description of work, contract value, completion date, owner contact, and explanation of similarity to this Project;

• Description of specialty artwork-installation experience;

• Direct-workforce and supervision plan;

• Rigging, lift, and equipment qualifications;

• Finished-facility protection experience;

• Safety-record information required by Section 7, including the Bidder's experience-modification rate for each of the most recent three years for which available; any serious or willful OSHA or Cal/OSHA citations and workplace fatalities during the preceding five years; and a brief description of corrective action taken; and

• Project-coordination experience.

Unless separate County forms are provided in the bid package, the Bidder shall prepare its Contractor Qualification Statement and signed Self-Performance Certification in its own format. The Qualification Statement shall address each applicable requirement of this Attachment. The Self-Performance Certification shall expressly address all certifications required by Section 2. The Bidder shall also complete and sign Section 11 of this Attachment.

Submission of a proposed construction subcontractor is inconsistent with the material self-performance requirement and may result in rejection of the Bid.

The Bidder shall certify that the submitted information is accurate and complete and shall authorize the County to verify licenses, registrations, certifications, project references, safety information, employment relationships, and other qualification information.

The Bidder shall promptly notify the County in writing if any submitted qualification information materially changes before Contract award.

11. BIDDER CERTIFICATION

I certify under penalty of perjury that the information submitted under this Attachment is accurate and complete; that the Bidder will self-perform the Work as required; and that the County may verify the information with licensing, registration, safety, employment, and project-reference sources.
