About this file
This is an applicant information and certification form for Request for Applications (RFA) 9288 issued by the Oregon Housing and Community Services (OHCS) to establish a Qualified Appraiser List for Multifamily Affordable Housing Development. The application requests qualified appraisers to apply for approval to provide appraisal services for OHCS affordable housing projects. Applicants must complete the comprehensive application package and submit all supporting documentation to DeAundra Lewelling at deaundra.lewelling@hcs.oregon.gov by the closing deadline specified in the RFA. The application process includes verification of applicant information, professional licensing, organizational structure, and compliance certifications. Approval is valid for two years and requires renewal; approved appraisers must execute an Appraiser Services Agreement (Attachment H) and may be removed from the approved list for failure to maintain professional standards.
Applicants must demonstrate an Oregon Certified General Appraiser license in good standing, professional liability insurance with current certificates, and compliance with federal and state tax obligations and business registration requirements. The application requires disclosure of conflicts of interest with OHCS staff, housing developers, nonprofits, and other entities, as well as certification of background and disciplinary history over the preceding five years. Applicants must verify non-debarment status in the System for Award Management (SAM.gov) and comply with all applicable federal and state laws including ORS Chapters 456, 458, and 279B, OAR Chapters 813 and 125, USPAP standards, and OHCS policies. Small business classifications and diversity certifications should be noted if applicable. No pricing information, funding amounts, or set-asides are specified in this application form.
