About this file

This is a Past Performance Questionnaire template designed for the Department of Veterans Affairs to evaluate contractor performance on previous contracts. Contractors must complete the General Information section (company name, address, project details, contract period, dollar value, and contractor role) and distribute the questionnaire to previous clients who served as references. Reference contacts submit completed forms directly to the VA Contracting Officer (Aldwyn Singleton at aldwyn.singleton@va.gov) and Ana Vazquez (ana.vazquez2@va.gov) by the solicitation due date, with the subject line referencing "Solicitation 36C24825R0044/Past Performance/Company Name."

The questionnaire evaluates contractor performance across multiple dimensions using a 1-5 rating scale (5=Exceptional, 4=Very Good, 3=Marginal, 2=Unsatisfactory, 1=Neutral/No Record). Assessment areas include management (experienced personnel, hiring practices, onsite supervision, government cooperation, employee conduct, subcontractor management); quality control (adherence to plans, material/equipment quality, timely deficiency correction, submittal accuracy); regulatory compliance (adherence to drawings, specifications, VA Technical Information Library guidelines, design guidelines, material specifications); problem resolution effectiveness; schedule performance (meeting deadlines, timely submittal submission, progress reporting, punch list completion); and warranty response. Reference contacts must provide narrative explanations for any ratings of 3 or 2, identify the contractor's two best performance traits, recommend whether to award another contract, and indicate CPARS rating status. Responses require the client's printed name, signature, and date.
