About this file

The document is a Past Performance Questionnaire (PPQ) for a Training and Instructional Support Services contract (RFP # 1333BJ25R00150009) issued by the United States Patent and Trademark Office (USPTO). The PPQ is a comprehensive evaluation form designed to assess contractor performance across six key areas: Technical/Quality of Service, Schedule/Timeliness, Cost Control, Management/Business Relations, Small Business Subcontracting, and Regulatory Compliance. The form provides detailed rating guidelines ranging from Exceptional to Unsatisfactory, with each category requiring a narrative explanation for ratings other than "Not Applicable".

The related federal contract opportunity indicates this is a competitive solicitation conducted under FAR Part 12 and Part 15, with award to be made on a best value basis. The USPTO anticipates awarding a Firm Fixed Price contract for Training Support Services, with a draft solicitation originally posted on March 21, 2025. Key changes from the draft include modifications to the project scope, place of performance, reporting requirements, invoicing, and CLIN structure. References are required to email completed PPQs directly to specified USPTO contacts (robert.garske@uspto.gov and kelly.gustafson@uspto.gov) with a specific subject line format.
