About this file

This is a Past Performance Questionnaire template issued by the Department of Veterans Affairs National Acquisition Center for Solicitation #36C79126R0003 – HAPA Commercial Requirements, which concerns the procurement of assistive listening devices.

The questionnaire must be completed and submitted by email to Bradley Geghan (bradley.geghan@va.gov) no later than the proposal receipt date specified in Block 8 of the SF-1449. It is the responsibility of the offeror to ensure timely distribution and submission of questionnaires by the company providing the past performance reference. The form requests basic contract information including the contractor's name, reference company/organization, contract number, performance location, period of performance, awarded amount, and contractor role (prime or subcontractor). Respondents must provide adjectival ratings (Excellent, Very Good, Satisfactory, Marginal, Unsatisfactory, or Neutral) across six evaluation categories: compliance with contractual requirements, adherence to performance schedule and quality control, contractor responsiveness to issues, effectiveness in addressing unforeseen customer needs, small business subcontracting plan performance (if applicable), and overall contractor satisfaction. The questionnaire also requires detailed descriptions of the contract scope, complexity, type, size, and schedule/corrective actions. Responses should include supporting comments for any significant discrepancies or excellent performance. Per Federal Acquisition Regulations, contractors may be notified of adverse remarks and given an opportunity to respond, though the evaluator's identity and completed questionnaires will not be disclosed to contractors or non-evaluation parties.
