About this file
This document is Appendix E of a Request for Proposal (RFP-2027-DBH-21-GETTI) issued by the New Hampshire Department of Health and Human Services for the Getting to Y (GTY) program. The RFP solicits responses to five mandatory technical questions from qualified vendors seeking to implement and manage the GTY program statewide. Respondents must describe their organization's experience with Youth Risk Behavior Surveys, learning collaboratives and train-the-trainer models, recruitment and collaboration with service-providing organizations, and knowledge of program fidelity requirements. Vendors must submit a detailed Implementation Plan that outlines a timeline with benchmarks, tasks, accountability measures, strategies, activities, deliverables, and proposed performance measures demonstrating service effectiveness across all State Fiscal Years. Additionally, vendors must provide a Recruitment and Collaboration Plan detailing how they will work with Regional Public Health Networks (RPHNs) to recruit middle and high schools for GTY programming, identify key personnel responsible for recruitment activities, and establish completion timelines and financial incentive strategies including stipends.
The RFP requires vendors to present a comprehensive Training and Technical Assistance (T/TA) Plan that ensures GTY program implementation maintains fidelity to the original design while complying with the UP for Learning GTY Train-the-Trainer model core practices. Respondents must identify the T/TA strategies and activities that will enable participating RPHNs and schools to learn and replicate the GTY program, designate key personnel responsible for T/TA delivery, and provide a schedule for completion. Vendors must also demonstrate organizational capacity to perform the entire scope of work by submitting an organizational chart with full-time equivalent staff (FTE) counts, qualifications and resumes for key personnel, and documentation of specialized staff training relevant to GTY program delivery. There is no page limit specified for Appendix E responses or associated attachments, and vendors must reference Appendix A (Scope of Services), Appendix F (Program Staff List), and the UP for Learning GTY resource materials when developing their proposals.
