About this file
This is a Quality Assurance Surveillance Plan (QASP) for a Fire Protection and Life Safety Training Contract. The QASP outlines how the government will monitor and evaluate contractor performance through systematic quality assurance methods.
The plan establishes specific surveillance items including: attendance sheet submission within 3 days of course completion, course completion reports within 30 days containing final grades and test materials, student critiques with a 95% collection requirement, key personnel qualifications and continuity requirements, periodic audits of course content and instructor competency, and invoice submission within 5 working days after travel. The QASP designates the Contracting Officer's Representative (COR) as responsible for technical administration and surveillance, utilizing methods such as customer feedback, periodic audits, and routine inspections. Performance standards generally require 0% error rates for accuracy and timeliness across most deliverables. The COR must maintain complete quality assurance documentation for the life of the contract.
