About this file
This is a Past Performance Questionnaire (PPQ) template and submission guidance document for the USCGC WAESCHE FY27 Quarter 2 Dockside Repair solicitation (Solicitation No. 70Z08526QLREP0020). The document establishes the framework for evaluating contractor past performance as part of the proposal evaluation process for this U.S. Coast Guard repair project.
The questionnaire requires offerors to submit completed CPARS (Contractor Performance Assessment Reporting System) evaluations when available; if CPARS evaluations are unavailable, offerors must distribute the PPQ template to previous customers for completion. Customers must evaluate contractor performance across three rating categories—Quality of Product or Service, Timeliness of Performance, and Business Relations—using a six-point scale (0=Unsatisfactory through 5=Outstanding). Completed questionnaires must be submitted directly by customers via email to Jaime Diaz (jaime.diaz3@uscg.mil) with a courtesy copy to Andrew Jacobs (Andrew.G.Jacobs@uscg.mil), referencing the solicitation number and company name in the subject line. Offerors are responsible for ensuring questionnaires arrive before the solicitation close date and time of 27 August 2026 at 2:00 pm Pacific Time; submissions received after this deadline will not be considered. The template collects standard contract information including offeror firm name, period of performance, customer contact details, contract award date, final contract value, contract type, and requirement description.
