About this file
This is a Past Performance Questionnaire template for evaluating contractor performance on Closed-Circuit Television (CCTV), Intrusion Detection System (IDS), and Access Control System (ACS) maintenance services at the Defense Finance and Accounting Service-Indianapolis, IN (DFAS-IN) under Solicitation HQ042326QE076.
The questionnaire is divided into three sections: Section A captures quoter information including company name, CAGE code, point of contact, contract details, and a description of work performed; Section B collects respondent company information; and Section C requests past performance ratings using a six-point scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Neutral) across four performance objectives—Overall Technical Performance, Quality of Work, Schedule Performance, and Cost Performance. The questionnaire also includes yes/no questions regarding contractor management capabilities, personnel qualifications, deficiency resolution, schedule development and compliance, and willingness to re-engage the contractor. Respondents must submit completed questionnaires to Coleman Dieger (dcoleman.dieger.civ@mail.mil) and Terri Davis (terri.l.davis34.civ@mail.mil) by 4:00 PM EST on July 27, 2026. Narrative explanations are required for any marginal or unsatisfactory ratings and any negative responses to the yes/no questions.
