About this file

This is a Contractual Acceptance Form for Invitation to Bid (ITB) S-47100-00015307 issued by the State of Oregon Employment Department (OED) for roof maintenance and repair services at the OED Eugene Field Office. The solicitation seeks qualified bidders to provide ongoing preventative maintenance and repair services, including semi-annual detailed inspections of the roof envelope, surface cleaning, and snow removal as needed to maintain roof system integrity. OED intends to award one contract through this competitive bidding process. The form requires bidders to submit comprehensive information including legal business name, primary contact details, business designation, federal tax identification number, and Oregon business registration number, along with certification of resident bidder status and any applicable Oregon certified firm designations (MBE, ESB, WBE, or DBE). Bidders must provide authorized representative contact information and complete all required documentation by the bid closing deadline.

The form establishes that bid pricing and costs shall remain valid for 30 days after the bid closing date or until contract approval, whichever occurs first. Bidders must certify compliance with non-discrimination employment practices, Oregon tax law compliance, and adherence to the Oregon False Claims Act. All bidders are required to acknowledge receipt of any addenda issued and confirm independent bid preparation without collusion or fraud. The form emphasizes that selected contractors must meet the highest standards prevalent in the roof maintenance and repair industry as outlined in the scope of work, and successful bidders will be bound by all stated terms and conditions upon contract award.
