About this file

This document is an Addendum to RFO 52.212-1 that supplements instructions to offerors for a Request for Quotations (RFQ) issued by the 36th Contracting Squadron at Andersen Air Force Base, Guam. The Government intends to establish multiple Blanket Purchase Agreements (BPAs) for the lease of portable toilets, hand wash stations, and restroom trailers. Quotations are due by 15 September 2026 at 1200pm and must be submitted to coree.asprer.1@us.af.mil and nathaniel.entilla@us.af.mil. The solicitation number is FA524026Q0030, and questions must be submitted by 08 September 2026 at 1200pm. A binding contract is formed only when individual call orders are placed and accepted by the awardee; quotations and BPAs themselves are not binding contracts.

Quoters may submit partial quotations for only those unit types, configurations, durations, and accessories they can furnish; unpriced items are treated as not offered. All quotations must include company information, completed pricing on the Schedule of Items, technical capability statements confirming units meet Statement of Work requirements with inclusive weekly service visits, delivery and servicing locations, destructive weather acknowledgment, and active SAM registration with current representations and certifications. Lease rates must be inclusive of weekly service visits per SOW Section 5.1 and transportation to the delivery destination. The Government reserves the right to add new BPA holders periodically through an on-ramp provision that does not affect existing BPA holders or previously placed orders. Award is contingent on the Contracting Officer's determination that the quoter is responsible, including demonstrated ability to comply with required delivery and performance.

Addendum to RFO 52.212-1, Instructions to Offerors — Commercial Products and Commercial

Services Multiple-Award BPA — Portable Toilet Leases

36th Contracting Squadron, Andersen AFB, Guam

1.0 Relationship to RFO 52.212-1

This addendum supplements the provision at RFO 52.212-1, Instructions to Offerors — Commercial Products and Commercial Services. Where this addendum and the basic provision differ, this addendum governs to the extent of the tailoring permitted by RFO 12.205(a)(1). The basic provision otherwise remains in full effect, including the paragraph on Debriefings, which is not tailored or removed.

2.0 Solicitation Type and Resulting Award

This solicitation is a Request for Quotations (RFQ) issued under the simplified procedures at RFO 12.201-1. The Government intends to establish multiple Blanket Purchase Agreements (BPAs) for the lease of portable toilets, hand wash stations, and restroom trailers, as described in the Statement of Work (Attachment 1) and the Schedule of Items (Attachment 2).

A quotation is not an offer. Neither a quotation nor any resulting BPA is, by itself, a binding contract. A binding contract is formed only when an individual call order is placed and accepted by the awardee (RFO 12.201-1(b)).

3.0 Partial Quotes Authorized

Quoters are not required to offer every item in the Schedule of Items to be considered for award. A quoter may price only those unit types, configurations, durations, and accessories it is able to furnish from current capability or inventory. The Government encourages quotes from all qualified sources, including those able to furnish only a portion of the Schedule of Items. Items left unpriced will be understood as not offered.

4.0 Content of the Quotation

Each quotation shall include the following. Quoters should keep submissions concise; elaborate formats are neither required nor desired.

Volume / Item Content

1. Quotation cover / quoter information

Company name, UEI, CAGE, address, point of contact, and SAM registration status.

2. Completed Schedule of Items

Pricing entered only for the items, unit types, durations, and accessories the quoter offers. Unpriced items are treated as not offered.

3. Technical capability statement

Brief description of the units offered and confirmation they meet the Statement of Work descriptions for each item priced, including that Schedule of Items Section 1.0 lease rates are inclusive of the weekly service visit described in SOW Section 5.1, and that units can be anchored, ballasted, or strapped in place appropriate to the site surface and local wind conditions.

4. Servicing and delivery statement

Locations the quoter can deliver, place, service, and remove units (Andersen AFB and any other identified locations), and the unit types deliverable to each.

Volume / Item Content

5. Destructive weather acknowledgment

A brief statement acknowledging the destructive weather responsibilities at SOW Section 4.6, including removal of units ahead of the installation's declaration of TCCOR 2 as the default protective action. The written severe weather plan is furnished after award upon Government request; it is not required with the quotation.

6. Representations and certifications

Active SAM registration with current representations and certifications (52.204-7), plus any individual provisions identified in the solicitation. The Government will rely on the quoter's SAM record; no consolidated reps-and-certs provision is submitted separately.

5.0 Preparation of the Schedule of Items

• Enter pricing only for items offered; leave non-offered items blank.

• Grey-filled cells do not apply to that line item and shall not be completed.

• Complete the duration columns (daily, weekly, monthly, annual) that apply to each offered item in

Schedule of Items Section 1.0; a blank duration column means that term is not offered for that item.

• Schedule of Items Section 1.0 lease rates shall be inclusive of the weekly service visit described in

SOW Section 5.1. A rate quoted without the weekly service visit is not comparable to other quotations and may be found unacceptable.

• Enter one-time charges for delivery, placement, and removal, additional servicing, and accessories in the One-Time column where indicated.

• State the minimum rental period, delivery lead time, and any extended-term discount in the Vendor Terms block at Section 5.0 of the Schedule of Items.

• Pricing shall be inclusive of transportation to the delivery destination consistent with RFO 12.203(a)(3). Do not embed prompt-payment discounts in the item prices.

6.0 Submission Instructions

Quotations shall be submitted as stated below. Late quotations will be treated in accordance with paragraph (f) of RFO 52.212-1.

• Due date and time: 15 September 2026, 1200pm.

• Method of submission: submit quotes to coree.asprer.1@us.af.mil and nathaniel.entilla@us.af.mil

• Solicitation number to reference: FA524026Q0030

• Questions are due by 08 September 2026 1200pm; submit to emails listed above. Answers will be provided by amendment.

7.0 Open Vehicle / Addition of Holders (On-Ramp)

The Government intends to keep the resulting multiple-award BPA open to additional qualified sources during its period. The Government may add new BPA holders periodically, at the Contracting Officer's discretion and generally in conjunction with the Government's annual review of the BPA. New sources will be evaluated against the same terms, conditions, and Schedule of Items applicable to the original awardees, and an added source executes the identical BPA. Adding a holder will not modify, disturb, or mailto:coree.asprer.1@us.af.mil mailto:nathaniel.entilla@us.af.mil

Attachment 4 — Addendum to RFO 52.212-1 — Page 3 of 3 recompete the BPAs of existing holders and will not affect orders already placed. This solicitation, or a successor notice, may be re-advertised periodically for this purpose.

8.0 Responsibility

Award of a BPA is contingent on the Contracting Officer's determination that the quoter is responsible, including the ability to comply with the required delivery and performance for the items offered.

Information bearing on responsibility (including past and current performance) may be considered for this purpose.
