About this file
This is a semi-annual safety device testing documentation template for the Miami Veterans Affairs Medical Center (VAMC) Boiler Plant. The document provides standardized checklists and procedures for testing and certifying ancillary equipment and safety devices required under VHA Directive 1810, with procedures effective January 1, 2024 through December 31, 2028.
The template covers fourteen semi-annual safety device tests identified by procedure numbers SD6MSOP 00 through SD6MSOP 14, including: plant reliability, high and low water alarms on condensate and deaerator tanks, deaerator overflow drain systems, deaerator safety valves, steam temperature and pressure comparisons, safety valves following pressure reduction valves, combustible gas alarms, outside air damper alarms, emergency stop buttons, and fuel pressure alarms. Each test includes detailed checklists organized by location and plumbing specifications, switch setpoint and mechanics requirements, logic verification steps, and provisions for documenting pass/fail results and required repairs. The documentation requires certification by the testing technician with their signature and date, confirming that all tests were performed per standard operating procedures and that no failures meeting the criteria in the Safety Device Testing Manual 7th edition were identified. The template mandates that all questions be answered affirmatively and specifies repair requirements for any failed components before the equipment is deemed acceptable.
