About this file

This is a First Quarterly Groundwater Monitoring Report for the Bruce W. Carter Veterans Affairs Medical Center in Miami, Florida, prepared by Tierra Group on behalf of the Department of Veterans Affairs. The facility, located at 1201 NW 16th Street, Miami, is subject to groundwater monitoring and assessment directed by Miami-Dade County Division of Environmental Resources Management (DERM) under regulatory file DERM UT-2694/File-0751 and FDEP No. 13-8628963.

Groundwater samples were collected on September 9, 2024, from monitoring wells MW-3 and MW-8 and analyzed by Pace Analytical Services for volatile organic halocarbons (VOHs), total recoverable petroleum hydrocarbons (TRPH), and arsenic. Results indicated total arsenic concentrations of 2.4 micrograms per liter (ug/L) at MW-3 and 8.8 ug/L at MW-8, with dissolved arsenic at 8.9 ug/L at MW-8 and chloroform detected at 1.0 ug/L at MW-8. All detected parameters were reported below their respective maximum contaminant levels (MCLs) and groundwater cleanup target levels (GCTLs). The report includes supporting laboratory analytical documentation, groundwater sampling logs, chain of custody records, calibration logs, and regulatory correspondence from June 17, 2024, approving the Groundwater Sampling Plan. Based on the results, continued quarterly groundwater monitoring is recommended in support of No Further Action with Controls (NFAC) status under Chapter 62-780.680, F.A.C.

GROUNDWATER MONITORING REPORT

DERM UT-2694/File-0751

1.0 INTRODUCTION

Tierra Group (Tierra) has prepared this First Quarterly Groundwater Monitoring Report for the

Bruce W. Carter Veterans Affairs Medical Center facility (DERM UT-2694/File-0751; FDEP No.

13-8628963), addressed as 1201 NW 16th Street, Miami, Florida, on behalf of the Department of

Veterans Affairs, Miami Veterans Affairs Healthcare System. The USGS Topographic Quadrangle

Map [Miami, Florida-Dade Co.] depicting the location of the facility is included as Figure 1.

According to the Miami-Dade County Property Appraiser, United States of America VA Hospital, is the legal owner of the subject property. Miami-Dade County Division of Environmental

Resources Management (DERM) is directing the assessment and monitoring at the site. This report summarizes the first quarter of groundwater monitoring activities performed following approval of the Groundwater Sampling Plan by Miami-Dade County DERM on June 17, 2024 (Appendix

A). Tierra conducted all sampling in accordance with the Florida Department of Environmental

Protection (FDEP) Standard Operating Procedures for Field Activities (DEP-SOP-001/01).

2.0 SITE DESCRIPTION

The site is situated in Section 35, Township 53 South, Range 41 East in Miami-Dade County at the junction of NW 16th Street and NW 14th Avenue. The site is currently occupied by the Bruce

W. Carter Veterans Affairs Medical Center, which spans 26 acres measuring approximately 900 feet from north to south and 1,200 feet from east to west. A Site Plan depicting the location of groundwater monitoring wells is included as Figure 2.

According to historical records, the facility was the subject of previous environmental evaluations, cleanup activities, and monitoring related to petroleum releases from underground storage tank

(UST) and aboveground storage tank (AST) systems. Site assessment activities included installing monitoring wells, evaluating groundwater flow conditions, and assessing contaminants of concern

First Quarterly Groundwater Monitoring Report Page 2

DERM UT-2694/File-0751; FDEP No. 13-8628963 in soil and groundwater. Prior recommendations included quarterly groundwater monitoring and excavation of impacted soils to facilitate site closure. During tank closure activities performed between May and June 2018, a total of 98.92 tons of impacted soil were removed from the UST excavation and properly disposed of at a permitted waste disposal facility. Further assessment data indicated that soil impacts remain beyond the footprint of the UST area.

3.0 GROUNDWATER SAMPLING AND ANALYSIS

On September 9, 2024, groundwater samples were collected from monitoring wells MW-3 and

MW-8. Groundwater samples were submitted to Pace Analytical Services, a National

Environmental Laboratory Accreditation Program (NELAP) accredited environmental laboratory

[Florida DOH certification number E83079], for analysis of volatile organic halocarbons (VOHs), total recoverable petroleum hydrocarbons (TRPH), and arsenic. Due to elevated turbidity after purging five well volumes from MW-8, an additional unpreserved sample was collected and requested for laboratory filtration and analysis of dissolved arsenic. Groundwater samples were collected in accordance with DEP-SOP-001/01 FS 2200, as provided by Chapter 62-160, Florida

Administrative Code (F.A.C.).

Review of groundwater analytical results yielded the following observations:

• Total arsenic was detected at a concentration of 2.4 micrograms per liter (ug/L) at MW-3.

No other parameters were detected above laboratory method detection limits (MDLs);

• Total arsenic and dissolved arsenic were detected at MW-8 at concentrations of 8.8 ug/L and 8.9 ug/L, respectively, demonstrating that turbidity did not contribute to the total arsenic concentration. Chloroform was detected at a concentration of 1.0 ug/L at MW-8.

No other parameters were detected above laboratory MDLs; and

• All parameters detected during the first quarterly groundwater monitoring event were reported at concentrations below their respective maximum contaminant levels (MCLs) and groundwater cleanup target levels (GCTLs), as provided by Chapter 62-550 and

Chapter 62-777, F.A.C., and/or Chapter 24, Code of Miami-Dade County.

Depth-to-water measurements gauged at monitoring wells MW-3 and MW-8 are included in Table

1. Groundwater analytical results are summarized in Table 2 and are depicted on Figure 3.

Groundwater sampling logs, calibration logs, and the laboratory analytical report with supporting chain of custody documentation are provided as Appendix B.

4.0 CONCLUSIONS AND RECOMMENDATIONS

Tierra Group prepared the First Quarterly Groundwater Monitoring Report for the Bruce W. Carter

Veterans Affairs Medical Center in Miami, Florida, on behalf of the Department of Veterans

Affairs, Miami Veterans Affairs Healthcare System. The report summarizes groundwater monitoring activities conducted following the approval of the Groundwater Sampling Plan by

Miami-Dade County DERM in June 2024. Prior assessment at the site demonstrated elevated concentrations of VOHs, TRPH, and arsenic, leading to recommendations for quarterly groundwater monitoring and soil excavation activities.

In September 2024, groundwater samples were collected from monitoring wells MW-3 and MW-

8 for analysis of VOHs, TRPH, and arsenic. Analytical results indicated total arsenic concentrations of 2.4 ug/L at MW-3 and 8.8 ug/L at MW-8. Dissolved arsenic was detected at

MW-8 at a concentration 8.9 ug/L, demonstrating that turbidity did not contribute to the total arsenic concentration. Chloroform, a common residual product of chlorinated potable water, was detected at 1.0 ug/L at MW-8. All detected parameters were reported below their respective MCLs and GCTLs.

Based upon results of the first quarterly groundwater sampling event, Tierra recommends continued groundwater monitoring in accordance with the approved Groundwater Sampling Plan, dated May 24, 2024, in support of No Further Action with Controls (NFAC) status under Chapter

62-780.680, F.A.C.
