About this file
This is a Past Performance evaluation attachment for a Request for Quotation (RFQ) for Chiller Coil Replacement services issued by the Department of Agriculture Agricultural Research Service. Quoters must submit two to three past performance projects completed within the last three years that are similar or larger in scope and complexity to the requirements outlined in the Statement of Work. At least two references must demonstrate the quoter's experience as a prime contractor, with additional references potentially from subcontractor roles. Each past performance submission must include the project title, description of scope and responsibilities, contract/order/BPA number, contract type and period of performance, contract award amount, government agency, current contact information for both the Contracting Officer and Contracting Officer's Representative, project status, and a brief narrative explaining the relevance to the current effort. The Government emphasizes that current point of contact information is critical, as outdated information may adversely impact evaluation. Past performance will be evaluated on a Go or No-Go basis, assessing the probability of the contractor's success in delivering services that meet the Government's needs based on demonstrated performance history.

Text version
Past Performance The past performance evaluation factor assesses the degree of confidence the Government has in an Offeror’s ability to provide services that meet Users’ needs, based on a demonstrated record of performance.
Quoters shall provide two (2) to three (3) projects describing their past performance on the projects under contract vehicles similar (or larger) in size, scope, and complexity to those requirements outlined in this RFQ. Recent is defined as within the last three years. Relevant is defined as work similar in complexity and magnitude of the work described in this Statement of Work.
Past Performance Submissions should include the following information:
§ Project title § Description of the project, scope, and responsibilities § Contract/Order/BPA number § Contract type and period of performance
· Contract Award Amount § Government Agency/Organization § COR’s name, address, and phone number § Contracting Officer’s name, address, and phone number or email address § Status (e.g. complete, ongoing, etc.)
§ A brief narrative of why the reference is relevant to this effort
At least two of the references provided shall be based on the Quoter’s performance as the prime contractor, and the others may be from subcontractors. Please make sure the point of contact information is current – failure to provide current information may have an adverse impact on the Government’s ability to evaluate your quote.
The Government may also consider information obtained through other sources. Past performance information will be utilized to determine the quality of the contractor’s past performance as it relates to the probability of success of the required effort and will be given a Go or No-Go rating.
