About this file

This is an Offeror Representations and Certifications checklist for the HRSA/MCHB Newborn Screening Systematic Evidence Review IDIQ solicitation #75R60226R00026. Offerors must complete and submit the checklist as part of their Volume III Administrative/Price submission, with inaccurate or incomplete certifications potentially rendering proposals unacceptable and ineligible for award. The checklist requires offerors to certify on six solicitation-specific items: (1) Business Size Status under NAICS code 541690 (Other Scientific and Technical Consulting Services), designating themselves as either Small Business Prime or Large Business Prime (with the latter requiring submission of a formal Small Business Subcontracting Plan); (2) whether they will provide covered telecommunications equipment or services under FAR 52.204-24; (3) organizational conflict of interest status relative to systematic clinical evidence reviews and policy recommendations for the Recommended Uniform Screening Panel, with potential conflicts requiring submission of a mitigation plan; (4) availability and commitment of all proposed Key Personnel, including a mandatory Principal Investigator with signed Letters of Commitment; (5) corporate capability to generate all digital deliverables in conformance with Section 508 accessibility standards; and (6) absence of unpaid delinquent federal tax liability or felony convictions under federal law within the preceding 24 months. An authorized signatory must sign and date the completed checklist with the offeror's name, UEI number, and TIN.
