About this file
This is a Past and Present Performance Questionnaire template used by a federal agency to evaluate contractor performance on design/build projects including new construction, renovation, alteration, and repair work with multiple disciplines.
The questionnaire is submitted to Athena.jackson@va.gov and collects general information including the contractor's name, project title, contract number, dollar amount, and performance period. Section B identifies the work scope as design/build projects with multiple disciplines. Respondents indicate their role (Prime Contractor, Sub-Contractor, Key Personnel, Teaming/Joint Venture, or Architect/Engineer) and provide contact information. The evaluation uses a five-point rating scale (1=Unsatisfactory, 2=Marginal, N=Neutral, 3=Satisfactory, 4=Very Good, 5=Exceptional) with "Not Applicable" as an option. Narrative explanations are required for any ratings of 1 or 2. The questionnaire contains 23 performance factors addressing management and staffing, delegation of authority, risk identification, responsiveness to changes, cooperation, quality control, subcontractor claims, site investigation, design quality, cost management, scheduling, environmental considerations, innovation, small business subcontracting implementation, deficiency correction, workforce quality, briefings, quality emphasis, cure notices, and likelihood of future contract award. Item 24 requests an overall rating on the 1-5 scale. The form includes a remarks section for additional narrative detail and notes that separate evaluation forms should be completed for other contracts held with the agency within the preceding five years.
