About this file
This is a Past Performance Questionnaire template used to collect contractor performance data for the Mississippi Group Optimized Remediation Contract (ORC) solicitation FA8903-26-R-0001. The questionnaire is to be completed by past customers or contracting officers of responding offerors and submitted directly to the Air Force Contracting Officer Nathan J. Fry (nathan.fry.7@us.af.mil) with courtesy copies to john.finkenstadt.1@us.af.mil and jose.hernandez.108@us.af.mil by January 26, 2026. The completed questionnaires are considered sensitive and will not be released to the offeror or other parties outside the government evaluation team.
The questionnaire evaluates contractor performance across four main areas: Program Management and Quality Assurance (including document quality, regulatory compliance, RCRA/CERCLA process implementation, project scheduling, and multi-site coordination); Regulatory Interaction and Stakeholder Coordination (including proactive regulatory engagement, state regulatory compliance, community involvement, contractor transition coordination, and multi-owner property management); Analysis and Data Management (including data management and reporting, risk assessment review, innovative approaches to cost reduction and site closure, data quality assurance, and remediation system design and optimization); and identification of any adverse performance issues. Each performance area uses a five-point rating scale: Exceptional (E), Very Good (VG), Satisfactory (S), Marginal (M), Unsatisfactory (U), or Not Applicable (N/A), with mandatory narrative explanations required for any Marginal or Unsatisfactory ratings.
