About this file
This is a California Civil Rights Laws Attachment form (DGS OLS 04, Rev. 01/17) issued by the California Department of General Services, Office of Legal Services, for RFP 25-3021 with the California Department of Social Services. The document serves as a mandatory certification requirement for any person or entity submitting a bid or proposal for state contracts valued at $100,000 or above. The attachment requires bidders to certify compliance with two primary civil rights statutes: the Unruh Civil Rights Act (Section 51 of the Civil Code) and the Fair Employment and Housing Act (Section 12960 of the Government Code). These certifications apply to all contracts executed or renewed after January 1, 2017.
The certification must be executed under penalty of perjury by an authorized official of the proposing or bidding firm and submitted at the time the bid or proposal is tendered or when a contract is renewed. Additionally, contractors must certify that any internal policies against sovereign nations or peoples recognized by the United States government are not applied in violation of the aforementioned civil rights laws. The form requires completion of standard identification fields including the proposer or bidder firm name, Federal ID number, authorized signature, printed name and title of the signing official, county and state of execution, and date executed. This certification attachment is a mandatory compliance document that does not involve pricing, set-asides, or other contractual terms.
