About this file
This document is a Sample Statement of Work (SOW) template for the New York State Office of Information Technology Services (ITS) under RFP #C000979 for Salesforce Professional Services Enterprise Agreement. The SOW serves as a framework for defining the scope, responsibilities, deliverables, and terms of engagement between ITS and a selected Salesforce-certified consulting partner. The template outlines the structure for project management, solution descriptions, key assumptions, vendor responsibilities, ITS responsibilities, estimated schedules, and completion criteria. The document is designed to be completed with specific project details, consultant qualifications, hourly rates, estimated effort hours, and total charges. Deliverables are subject to a five-business-day review and approval process by the ITS Project Manager, with provisions for requested revisions within an additional five-business-day timeframe. The SOW is executed pursuant to the base Enterprise Agreement No. C000979 and requires signatures from authorized representatives of both ITS and the vendor, along with notarization.
The pricing model for services under this SOW follows an hourly rate structure, with invoices submitted monthly for actual hours provided at specified hourly rates in accordance with New York State Finance Law and the electronic contract payments process outlined in Agreement C000979. Travel and living expenses are included within the hourly rates and charged according to published NYS contract guidelines. ITS retains the right to cancel the SOW without cause with reasonable written notice, with compensation limited to services received prior to cancellation. The document includes change control procedures requiring joint written documentation and approval from both parties, as well as a multi-level escalation procedure for conflict resolution between ITS and the vendor. ITS is responsible for providing office facilities, obtaining required consents for access to systems and data, maintaining data security and content integrity, and designating a Project Manager to serve as the focal point for all vendor communications and decisions.
