About this file
This is a Work Summary & Progress Report form for an Upright Headstone Raise & Realignment project under a National Shrine Project contract. The form serves as a reporting template for contractors to document completion of cemetery headstone work and is explicitly noted as not to be used as an invoice.
The form requires contractors to report on six key work completion items: (1) selection and installation of section keystones with COR approval using a licensed surveyor; (2) excavation of upright headstone sockets with minimum 3-inch clearance on all sides and bottom, including preparation of crushed stone base material; (3) installation of upright headstones to uniform height (25" above ground, or 20" for older/smaller headstones) with horizontal and vertical plumb alignment within 1/8" transverse and lateral tolerance and within one degree of vertical, with COR random sampling verification; (4) ensuring completed headstones are firmly set with no movement or play, verified through COR random checks; (5) achievement of neat overall appearance with vertical, lateral, and transverse alignment providing uniform flowing transition within and between sections; and (6) cleaning of headstones per contract requirements. For each item, contractors must document quantity, section numbers, and completion dates. The form includes signature lines for both contractor and Cemetery Director/Contracting Officer's Representative (COR), with a requirement that both parties acknowledge receipt of the completed progress report. The form may be duplicated as necessary for multiple reporting periods, and contractors must report any circumstances adversely affecting work progress such as weather, strikes, or government delays.
