About this file
This is a Performance Work Statement (PWS) exhibit containing referral guidelines for the Polytrauma/Brain Injury Medicine and Intrepid Spirit Center (ISC) at a military treatment facility. The document serves as a clinical decision-support tool for processing referrals for Active Duty Service Members (ADSM) with traumatic brain injury (TBI) diagnoses.
The guidelines establish acceptance criteria for referrals to the TBI and ISC clinic, including requirements that patients have a documented TBI diagnosis, demonstrate persistent or recurrent physical, cognitive, or behavioral symptoms, have received prior treatment without resolution, and maintain at least six months of active-duty status with availability for rehabilitation. The document defines acceptable diagnoses encompassing mild to severe TBI, subacute or chronic TBI with complex comorbidities, recurrent concussions (three or more within 12 months), and TBI-related conditions including behavioral health and chronic pain issues. Specific guidance is provided for processing different referral types, including 2nd/3rd opinions, follow-up care, referrals from external providers, and specialty services such as neuropsychology, occupational therapy, physical therapy, vestibular rehabilitation, and neuro-optometry. The guidelines establish closure criteria for referrals that do not meet clinical requirements, including acute head trauma cases (0-14 days), referrals from non-ADSM populations, and patients already established in the clinic within the past six months. The document includes booking instructions directing all referrals to clinic phone number 253-968-6478 and identifies key clinic providers and contact personnel for referral management questions and urgent review requests.
