About this file
This file is a Performance Work Statement (PWS) exhibit containing primary care appointing criteria and operational protocols for the Madigan Hearing Program at a Defense Health Agency Military Treatment Facility. The document serves as a template and reference guide for the Integrated Referral Management and Appointing Center (IRMAC) services contract, establishing standardized procedures for scheduling appointments and managing patient referrals.
The Madigan Hearing Program operates Monday, Wednesday, Thursday, and Friday from 6:45 AM to 3:30 PM, and Tuesday from 6:45 AM to 3:00 PM, located in Building 9911C (Old Madigan Annex). The clinic serves Active Duty personnel, Reservists/National Guard Service Members, and noise-exposed DoD/DHA civilian employees with no referral requirement. Two primary appointment types are available: Medical Readiness appointments (HEAR GRP/HEAR GRP 60) for MEDPROS follow-ups, annual physicals, and separation/retirement requirements, and Specialty appointments (HEAR SPEC/HEAR SPEC 60) for diagnostic hearing and tinnitus evaluations requiring prior DD2215/2216 hearing exam completion within three months. The clinic leadership includes MAJ Stacie Harber (Chief Physician) and 1LT Grace Barnhart (Deputy Chief Physician) for specialty appointments, with additional staff assigned exclusively to medical readiness scheduling. Patient instructions include arriving 15 minutes early, bringing valid identification and insurance cards, entering via ramp 2, using parking behind Fisher House 1, and noting that the Madigan Gate will be closed through early-to-mid spring 2026. Virtual appointments are available for eligible circumstances, and patients may submit Standby Appointment Requests when first-available slots are unavailable.
