PERFORMANCE WORK STATEMENT (PWS)

FOR CUSTODIAL SERVICES

1. DESCRIPTION OF SERVICES. The Contractor shall provide non-personal services to include all personnel, labor, equipment, tools, supervision, management and other items and services necessary to perform custodial services at Altus AFB. Services shall be performed in a manner that will maintain a satisfactory facility condition and present a clean, neat and professional appearance. The Contractor shall accomplish all facility, restroom and periodic cleaning tasks in order to meet the requirements of this PWS and the Service Summary (SS).

The Contractor shall be required to provide and replenish liquid soap, multi-fold and/or roll paper towels and toilet paper products during regular services and when notified of low stock by the Contract Officer Representative (COR). Cleaning frequencies are established in Appendix A and Air Force Common Levels of Service (AFCOLS). Facility cleaning levels and estimated square footages are established in Exhibit 2, Estimated Square Footages and Level of Service.

Facility drawings of areas cleaned are established in Appendix B, Floor Plans and Maps. Floor plans and maps will be provided to the Contractor upon contract award or as changes to square footage/buildings occur. All work performed by the Contractor shall be performed in accordance with (IAW) this PWS, all applicable laws, AF COLS, other instructions or regulations and commercial practices.

1.1. CLEANING, DISINFECTING AND SANITIZING SOLUTIONS. The Contractor is responsible to provide all cleaning, disinfection and sanitizing solutions. All chemicals must have HAZMAT approval (see section 4.4) prior to bringing the chemicals on base. Chemicals must be stored in the original container or a container clearly labeled with the contents (i.e. – “Bleach Water”). All cleaning, disinfecting and sanitizing solutions shall be kept out of the reach of children at all times.

1.1.1. CLEANING SOLUTION. Use soap and water or commercial germicidal detergent to clean visibly soiled areas. Use of a scrub brush or other tool is authorized as long as it will not mar or damage the surface being cleaned.

1.1.2. DISINFECTING SOLUTION. Use a chemical agent or commercial disinfectant to prevent infection from non-food surfaces and equipment by destroying harmful bacteria and other microbes. These chemicals must be approved for use on base, registered with the US EPA and marked on the label.

Examples:

- Liquid chlorine bleach (without scent), one-quarter (1/4) cup (5.25% hypochlorite solution) in one (1) gallon of cool water; or one (1) Tablespoons bleach in one (1) quart cool water, made fresh daily.

- Quaternary ammonium compound mix at 400 ppm or as directed, confirm with test papers.

- Pine oils (must be at least 70% pine oil) - follow manufacturer’s directions.

- Commercial disinfectants - follow manufacturer’s instructions.

1.1.3. SANITIZING SOLUTION. Use a limited list of chemical agents that reduce the number of bacteria and viruses to a safe level for public health requirements. Sanitizing solutions are used on any food preparation surfaces, food eating surfaces and other associated furniture (i.e. chairs, highchairs) or equipment. The crucial element to sanitizing is the length of time the solution is left on the surface.

Examples:

- Liquid chlorine bleach (without scent), one (1) Tablespoon (5.25% hypochlorite solution) in one (1) gallon of cool water, made fresh daily. Solution must be left on the surface for a minimum of two (2) minutes to be effective.

- Quaternary ammonium compound mix as directed and confirm with test papers. Check with label to ensure it is recommended for food surfaces. Ammonium compounds are deactivated by soap and detergents and may not protect against all bacteria. Follow manufacturer’s instructions and allow time to work.

1.1.4. CLEANING PRODUCTS APPROVAL. Chemicals must be approved by the Civil Engineer Squadron (CES) Environmental Flight prior to bringing them on base (paragraph 4.4.2.).

1.2. FACILITY CLEANING SERVICES. Facility cleaning services are performed on a regular recurring basis. These services include cleaning flooring, stairwells, elevator cabs and drinking/bottle filling fountains. These service requirements apply to all building level of service categories. Render the appropriate services for each flooring type. Floor types are subject to change without notice.

1.2.1. NON-CARPETED FLOORS (HARD FLOORS – VINYL TILE, CERAMIC TILE, WOOD,

CONCRETE, ETC.). This service includes sweeping and mopping of floor surfaces. The Contractor shall maintain hard floors free of grit, soil, dust, scuff and heel marks, stains, spills, debris, litter and other foreign matter by effective routine cleaning. Accomplish sweeping by the most appropriate method (vacuum, sweep, dust mop, etc.). Accomplish mopping by the most appropriate method with cleaning solutions for the specific floor type (damp mop, wet mop, scrape to remove grime, machine scrub, etc.). After cleaning, ensure floor surfaces (to include grout) have a uniform, clean appearance without streaks, swirl marks, detergent or soap residue or any evidence of soil, stain, film or standing water. Clean baseboards, corners and edges where the wall and floor meet. Surfaces from the floor to the top of the baseboards must be free of dust, lint, cobwebs and litter. Move chairs, trash receptacles and other easily moveable items to maintain floors underneath these items and return them to their original and normal location after cleaning.

1.2.2. CARPETED FLOORS. This service utilizes efficient vacuum cleaners that offer high airflow, high efficiency filtration and an adjustable rotating brush agitation for more effective soil removal. The Contractor shall maintain carpets to be free of soil, dirt, debris, litter and other foreign matter by effective routine vacuuming. Remove any spots and/or spills by the carpet manufacturer’s approved methods or other commercially accepted practices as soon as noticed (spot clean). Notify the Facility Manager or the Contracting Officer Representative (COR) of all tears, burns and raveling. Area and throw rugs shall also receive this service. Clean baseboards, corners and edges where the wall and floor meet. Move chairs, trash receptacles and other easily moveable items to maintain floors underneath these items and return them to their original and normal location after cleaning.

1.2.2.1. CARPET SPOT CLEANING. Carpet spot cleaning shall be performed any time a spot or stain (not to exceed one (1) square yard) is noticed during scheduled carpet vacuuming services. Remove the spot/stain using the carpet manufacturer’s approved methods or other commercially accepted spot/stain treatment practices. Notify the COR of the location of the spot/stain treatment and if removal was unsuccessful. The COR will advise if the Contractor should repeat the process to obtain a successful result (no more than 1 additional treatment for each spot/stain).

1.2.3. STAIRWAYS AND ELEVATORS. Clean floor surfaces IAW paragraph 1.2.1. and/or 1.2.2., as appropriate. Clean stair guards, handrails, wall caps and baseboards to be free of grease and grime. Remove all marks, dirt, smudges, scuffs and other foreign matter from elevator cab and adjoining stairwell walls, up to 6 feet high from floor or stair tread level, to provide or maintain a clean and uniform appearance.

1.2.4. CLEAN FLOOR WALK-OFF MATS. Vacuum and/or sweep clean interior floor mats.

Maintain mats free of all visible lint, litter, debris, soil and other foreign matter. Remove soil and moisture from underneath mats and return them to their normal location.

1.2.5. DRINKING FOUNTAINS. Clean and disinfect all facility drinking and water bottle filling station fountains. Clean all porcelain and polish metal surfaces, including the orifices and drain, as well as all exterior surfaces of fountains. Maintain drinking fountains free of streaks, stains, spots, smudges, scale and other obvious soil.

1.2.6. TRASH COLLECTION/REMOVAL. RESERVED

1.3. RESTROOM AND LOCKER ROOM CLEANING SERVICES. Restroom cleaning services are performed on a regular recurring basis. These services requirements apply to all building level of service categories. Each restroom shall have all services completed prior to moving to the next restroom to limit the time each restroom is out of service.

1.3.1. GENERAL RESTROOM AND LOCKER ROOM TASKS. The tasks listed below shall be accomplished with each service for all Levels of Service (Level 1 through Level 4). The specific frequency for each level of service can be found in Appendix A.

1.3.1.1. FLOORS. Floors shall be swept AND mopped maintain the floors free of grit, soil, dust, stains, debris, and other foreign matter. Move trash receptacles and other items on the floor or tilt all moveable items to sweep and clean with solutions for the specific floor type. Maintain grout on floor tiles to be free of dirt, scum, mildew and residue. If carpet is present, carpet shall be vacuumed and trash receptacles and other items will be moved to vacuum under these items.

Baseboards, wall edges where the floor meets the wall and under easily moved items shall be cleaned. Flooring shall be cleaned concurrent with restroom services.

1.3.1.2. SHOWERS (IF PRESENT). Clean and disinfect shower walls, doors, shower curtains, shower mats and fixtures to be free of water spots, scale build-up and soap scum. Shower curtains shall be cleaned and free from dirt and mold. Drains shall be clean and free of hair and other foreign matter. Drains shall have no odor or have a clean scent and shall drain freely. If drains are clogged, notify the facility manager or COR.

1.3.1.3. SINK AND VANITY AREA. Clean and disinfect all sinks, plumbing fixtures, countertops and surrounding cabinetry. Clean mirrors at sink/vanity area to be free of water spots, grime and streaks (no height limitation). Sink and Vanity area shall be free of grime, dust, water spots, scale build-up and soap scum. Clean and disinfect all surfaces of soap dispensers, paper towel dispensers, hand dryers and other dispensers in the restroom or locker room.

1.3.1.4. TOILETS AND URINALS (IF PRESENT). Clean and disinfect toilets, urinals and fixtures (handles, plumbing pipes) to be free of water spots and scale build-up. Toilet paper dispenser shall be cleaned and disinfected and free of grime, dust, water spots or streaks.

1.3.1.5. INFANT CHANGING TABLES (IF PRESENT). Infant changing tables shall be clean and disinfected to be free of grime, dust and other debris.

1.3.1.6. TRASH. Restroom trash shall be emptied and removed with each service. Contractor is responsible to replace all restroom trash can liners. Small trash cans in toilet stalls in female restrooms shall be emptied and cleaned with each service to be free of odor. Trash cans/ receptacles shall be cleaned and disinfected the first service of every week. Return trash can/receptacle to the original location.

1.3.1.7. CLEAN WALLS, PARTITIONS AND DOORS. Clean toilet/urinal stall partitions and doors. Clean shower stall partitions and doors if present. Clean entry doors to the restroom.

Clean all handles, push guards and kick plates on doors. Walls shall be cleaned to 6 feet from the floor. All door handles, push plates, kick plates and ventilation grates shall be cleaned and disinfected. Walls, partitions and doors shall be free of water spots, dirt and scum. Task shall be completed during the first service of each week.

1.3.1.8. DESCALE TOILETS, URINALS AND SHOWERS. Contractor shall use an approved chemical for descaling toilets, urinals and showers. Descale during the first 5 duty days of each month.

1.3.1.9. POWER SCRUB FLOORS. Contractor shall power scrub restroom and locker room floors. Baseboard, floor grout, corners, wall/floor edges and behind doors shall be free of dirt, scum, soap film. Power scrub floors during the first 5 duty days of each month.

1.3.1.10. RE-SUPPLY PAPER AND SOAP. Contractor is responsible to provide toilet paper and paper towels in the dispensers. Fill soap containers or dispensers at sinks. Soap shall not be watered down to extend stock. If supplies run out between services, the contract shall re-stock within 2 hours of notification from the COR at no extra cost to the Government. Contractor is responsible to provide all keys to access the toilet paper and paper towel dispensers in the restrooms.

1.3.1.11. COMPLETION OF SERVICES. Upon completion of Restroom Services, ensure that the restroom is free of odor and has a clean scent.

1.3.1.12. FLOOR DRAIN ODOR REMOVAL. A disinfecting solution may be used on the floor drains to remove odor. A liquid chlorine bleach solution of one-half (1/2) cup of bleach in 1 gallon of water may be used. If the solution does not remove the odor, notify the Facility Manager or the COR so that a work request may be turned in to have the drain inspected.

1.3.1.13. OCCUPIED RESTROOMS. Occupied restrooms require services prior to leaving the facility. Post appropriate signage alerting occupants that the restroom is “closed for cleaning.”

Do not close all facility restrooms simultaneously unless the facility contains only one restroom.

1.4. LEVEL 4 CLEANING TASKS. Level 4 cleaning tasks are for the Fitness Center, Youth Center and Child Development Center (CDC) ONLY. The following tasks occur on a recurring basis. Periodic tasks (quarterly, semi-annually and annually) are addressed in Section

1.5. Frequencies for each task for each location are located in Appendix A.

1.4.1. FITNESS CENTER SPECIFIC TASKS. The Fitness Center shall be maintained IAW DoDI

1015.10 DoD MWR Fitness Standards. In addition to the facility cleaning and restroom tasks that apply to all facilities, the following tasks shall be performed at the Fitness Center. All Tasks shall be accomplished after 4 p.m. unless the tasks has a frequency two times during one day.

For those specific cleaning requirements of 2 times daily, at least one of the services shall be performed during the duty day between 0900 and 1100. Contractor personnel shall use only the main entrance to enter and leave the building.

1.4.1.1. CLEAN WEIGHT SCALES. All surfaces of weight scales shall be cleaned and disinfected twice daily.

1.4.1.2. CLEAN MULTI-PURPOSE EXERCISE AND WEIGHT ROOM TASKS. The Contractor shall clean exercise equipment housing that is in contact with the floor. Rubber floors shall be swept and damp mopped, including baseboards, corners and wall edges. Exercise room mirrors shall be cleaned to be free of streaks. These tasks shall be performed daily.

1.4.1.3. CLEAN PARENT CHILD AREAS. Clean exercise equipment housing that is in contact with the floor. Clean mirrors to be free of streaks. Clean and disinfect children’s play area floor.

These tasks shall be performed daily.

1.4.1.4. CLEAN BASKETBALL AND RACQUETBALL COURTS. The Contractor shall clean interior glass (both sides) up to 6 feet at the racquetball courts. Wood floors shall be swept and damp mopped so that no moisture remains on the wood floors. These services shall be performed daily. Walls shall be spot cleaned up to 6 feet from the floor and padded walls around courts shall be cleaned during the first service of each week.

1.4.1.5. CLEAN BLEACHERS. The Contractor is not responsible to pull out or push back bleachers at the Fitness Center. The Contractor shall pick up debris on and around bleachers daily. Contractor personnel are not required to remove debris from under bleachers. Remove chewing gum from seats/bleachers. Sweep and mop bleachers, stairs and clean and disinfect bleacher seats during the first service of each week.

1.4.1.6. EMPTY TRASH. Trash cans in courts, exercise rooms, weight rooms, parent/child rooms, multi-purpose rooms and corridors/hallways shall be emptied daily. Clean and disinfect trash cans during the first service of each week.

1.4.1.7. CLEAN ENTRYWAY GLASS AND DOORS. Clean the interior and exterior of all entry way glass and doors. Glass shall be free of streaks. Doors shall be free of streaks and grime.

Push plates, kick plates and all handles shall be cleaned. This service shall be cleaned during the first service of each week.

1.4.1.8. LOW DUST RESTROOMS AND LOCKER ROOMS. Low dust surfaces within 6 feet of the floor. Surfaces must be free of litter, dust, lint and cobwebs. Surfaces could include storage lockers, window sills, pictures, window blinds, ceiling fans, air vents, etc. Low dusting shall be completed during the first service of each week.

1.4.1.9. CLEAN SAUNAS. The Contractor shall clean and disinfect the floor and benches daily.

Interior and exterior glass shall be cleaned and free of streaks. Walls shall be cleaned and disinfected during the first service of each week.

1.4.1.10. POWER SCRUB SHOWER FLOOR AND DRAIN COVERS. Contractor shall power scrub shower floors and drain cover to be free of dirt, scum, soap film. Power scrub shall be completed during the first service of each week.

1.4.1.11. SPOT CLEAN WALLS. Spot clean walls up to 6 feet from the floor. Restrooms and locker rooms shall be completed during the first service of each week. All other area, except administrative areas, shall be spot cleaned during the first 5 duty days of each month.

1.4.1.12. BUFF FLOORS. Hallway, corridor, lobby and entrance way floors shall be buffed and polished during the first 5 duty days of each month.

1.4.1.13. CLEAN ADMIN AREAS OF FITNESS CENTER. All offices, HAWC, front desk areas and equipment check out areas shall be cleaned weekly. Centralized trash collection shall apply to all Admin areas with the collection point at the front desk area. These tasks shall be completed during the first service of each week.

1.4.2. YOUTH CENTER SPECIFIC TASKS. Youth Center facilities shall be maintained IAW AFI 34-144, Child and Youth Programs. In addition to the facility cleaning and restroom tasks that apply to all facilities, the following tasks are performed at the Youth Center. All Tasks shall be accomplished after 4 p.m. unless otherwise arranged in advance by the COR. This notification must be made no less than 7 days in advance. Kitchen areas are not part of the Contractor’s responsibility. Cleaning equipment (mops, broom, cleaning supplies, etc.) shall be inaccessible to children at all times. Contractor personnel shall sign in and out at the front desk upon entry and exit and use only the main entrance to enter and leave the building. Physical contact shall not be allowed and Contractor personnel shall not be alone with children unsupervised.

1.4.2.1. CLEAN DOORS AND DOOR KNOBS. Clean doors and door knobs, including push plates, hand pulls and kick plates daily.

1.4.2.2. CLEAN AND SANITIZE FOOD AREAS. The Contractor shall clean tables and chairs used for eating after each meal. Contractor shall sanitize all tables and chairs after the last meal.

Clean and sanitize food preparation counters and surfaces, sinks (including plumping fixtures) and serving tables or surfaces daily. Clean and disinfect tables, chairs and floors in food-eating areas daily.

1.4.2.3. EMPTY AND DISINFECT TRASH CANS. The Contractor is responsible to empty trash cans at the Youth Center once daily. This task includes the outside trash located in the adjacent fenced playground area. Trash in the kitchen and other areas such as outdoor courts or fields, is excluded from this service. Clean and disinfect trash cans during the first service of each week.

1.4.2.4. COURTS. The Contractor shall clean padded walls around courts. Padded walls around courts shall be cleaned during the first service of each week. Wood floors shall be swept and damp mopped daily. No moisture shall remain on the wood floors.

1.4.2.5. CLEANING BLEACHERS. The Contractor is not required to pull out bleachers to perform this service. Contractor shall pick up debris on and around bleachers daily. Remove chewing gum from seats/bleachers. Sweep and mop bleacher stairs weekly. Clean and disinfect bleacher seats weekly. These tasks shall be performed during the first service of each week.

1.4.2.6. CLEAN GLASS AND MIRRORS. Clean interior glass and mirrors not in restrooms (up to 6 feet from floor) to be free of streaks. This service shall be performed during the first service of each week.

1.4.2.7. CLEAN CABINETS. Clean all exposed surfaces of cabinets and disinfect. Remove all dirt, grime and other marks. This service shall be performed during the first service of each week.

1.4.2.8. LOW DUST. Low dust surfaces within 6 feet of the floor. Surfaces must be free of litter, dust, lint and cobwebs. Surfaces could include surfaces such as tables, shelves, window sills, handrails, pictures and window blinds. This service shall be performed during the first 5 duty days of each month.

1.4.2.9. SPOT CLEAN WALLS. Spot clean walls up to 6 feet from the floor. Restrooms shall be completed during the first service of each week. All other area, except administrative areas, shall be spot cleaned during the first 5 duty days of each month.

1.4.2.10. BUFF FLOORS. Buff/polish floors in the Youth Center. This service shall be performed during the first 5 duty days of each month.

1.4.2.11. CLEAN ADMIN AREAS OF YOUTH CENTER. Administration areas shall receive Level 2 services (see Appendix 2 for frequency). All offices, front desk areas and other admin areas are included in this requirement. Centralized trash collection shall apply to all Admin areas with the collection point at the front desk area. Trash collection shall be completed during the first service of each week.

1.4.3. CDC SPECIFIC TASKS. The CDC facility shall be maintained IAW AFI 34-144 Child and Youth Programs and the National Association for the Education of Young Children (NAEYC) accreditation. In addition to the facility cleaning and restroom tasks that apply to all facilities, the following tasks are performed at the CDC. All Tasks shall be accomplished after 4 p.m. unless the tasks has a frequency two or three times daily. For those specific cleaning requirements one or two of the services shall be performed during the duty day. Sweeping and mopping of “eating” floors shall be accomplished after breakfast, lunch and pm snacks. Kitchen areas are not part of the Contractor’s responsibility. Cleaning equipment (mops, broom, cleaning supplies, etc.) shall be inaccessible to children at all times. Contractor personnel shall sign in and out at the front desk upon entry and exit and use only the main entrance to enter and leave the building. Physical contact shall not be allowed and Contractor personnel shall not be alone with children unsupervised.

1.4.3.1. CLEAN DOORS AND DOOR KNOBS. Clean doors and door knobs, including push plates, hand pulls and kick plates.

1.4.3.2. FOOD-EATING AREAS. Clean all tables, highchairs and chairs in food-eating areas after each meal. Tables, highchairs and chairs shall be sanitized after the last meal. The Contractor will sanitize all other remaining tables and chairs daily. The Contractor shall sweep and mop the floors in eating areas after each meal. Floors shall be disinfected after the last meal.

Frequencies are listed in Appendix A.

1.4.3.3. CLEAN AND SANITIZE FOOD AREAS. Clean and sanitize food preparation counters and surfaces, sinks (including plumping fixtures) and serving tables or surfaces daily.

1.4.3.4. CLEAN GLASS AND MIRRORS. Clean interior window, glass and mirrors not in restrooms (up to 6 feet from floor) to be free of streaks. This service shall be performed during the first service of each week.

1.4.3.5. CLEAN CABINETS. Clean cabinets to be free of dirt, grime and other marks.

Frequencies are listed under Appendix A.

1.4.3.6. EMPTY AND DISINFECT TRASH CANS. The Contractor is responsible to empty trash cans at the CDC daily. This task includes the outside trash located in the adjacent fenced playground area. Trash cans shall be cleaned and disinfected the first service of each week.

Trash in the kitchen and other areas such as outdoor courts or fields, is excluded from this service.

1.4.3.7. SPOT CLEAN WALLS. Spot clean walls, up to 6 feet from the floor, to reduce the presence of grime, dirt or other marks. This service shall be performed during the first service of each week.

1.4.3.8. LAUNDRY ROOM. Sweep the laundry room during the first service of each week.

Mop the laundry room every other week.

1.4.3.9. LOW DUST. Low dust surfaces within 6 feet of the floor. Surfaces must be free of litter, dust, lint and cobwebs. Surfaces could include surfaces such as tables, shelves, window sills, handrails, pictures and window blinds. This service shall be performed during the first 5 duty days of each month or as required.

1.4.3.10. BUFF FLOORS. Buff/polish floors in the CDC. This service shall be performed during the first 5 duty days of each month.

1.4.3.11. CLASSROOM RESTROOM CLEANING. Cleaning tasks for classroom restrooms shall be performed IAW the frequencies listed in Appendix A for the specific child-age room.

Restroom paper products and soap shall be re-stocked daily

1.4.3.12. CLEAN ADMIN AREAS OF CDC. All offices, front desk areas and other admin areas shall be cleaned every other week. Centralized trash collection point shall be at the front desk.

The trash collection shall be performed during the first service of each week.

1.4.4. TASKS SPECIFIC TO CHILDREN UNDER 24 MONTHS AREAS. The following tasks are specific to classrooms for children under 24 months.

1.4.4.1. CLEAN/DISINFECT DIAPER CHANGING TABLES AND ADJACENT WALLS. Clean and disinfect diaper changing counters, sinks and adjacent walls at least daily, and as needed, with hot water and a disinfecting detergent.

1.4.4.2. EMPTY AND CLEAN DIAPER PAILS. The Contractor is responsible to provide trash bags for diaper pails. Empty diaper pails twice daily. Clean and sanitize diaper pails with hot water and a disinfecting detergent after the last service.

1.4.4.3. CLEAN/DISINFECT CHILDREN’S TOILETS AND FIXTURES. Clean toilets, sinks, counters and adjacent walls (up to 4 feet from floor) with hot water and disinfecting detergent and then sanitize with the bleach water solution at least twice daily (after lunch and in the evening, after hours).

1.4.4.4. SWEEP/MOP CHILDREN’S BATHROOM FLOORS. Sweep and mop the children’s bathroom floors with hot water and detergent and then sanitize with bleach water solution.

Return all furnishings that were moved for cleaning, back to their original location and position after the floor is dry.

1.5. PERIODIC CLEANING SERVICES. Periodic cleaning services are those specified as “as needed” quarterly, semi-annually or annual services. These services shall be requested by the COR on a work order. If there are specific dates requested by the customer, that information shall be included on the work order. The COR shall request the service at least 10 days prior to the beginning of the service performance month. The Contractor shall use the work orders received from the COR to create a work schedule for the performance month. The Contractor shall forward the schedule by at least 2 days prior to the beginning of the performance month.

The Periodic Cleaning schedule is located in Exhibit 4. The Contractor shall notify the COR after completion of the periodic task so that the area may be inspected by the COR. The Contractor may request changes to this schedule within the first 60 days after contract award.

1.5.1. FLOOR BUFFING. Periodic floor buffing is used as an interim service to extend the floor finish until floor maintenance is required. Floors shall be cleaned prior to buffing. Move chairs, trash cans and other easily moveable items to perform the service. Floor shall have a uniform clean appearance without streaks, swirl marks or residue. Replace chairs, trash cans and other items to their original location.

1.5.2. PERIODIC HARD FLOOR MAINTENANCE. Floors shall be cleaned prior to performing floor maintenance. Move chairs, trash cans and other easily moveable items to perform the service. If applicable, existing floor wax shall be stripped from the floor using commercial practices and approved chemicals. Floors shall be scrubbed to remove all dirt, grime, stains and scuff marks. Wax shall be applied according to manufacturer recommendations. Once applied wax has dried, the floor shall be buffed to polish and seal the floor. Tile floors shall not be waxed. Tile floors and grout shall have a uniform clean appearance. Floor shall have a uniform clean appearance without streaks, swirl marks, residue or any evidence of soil, stain, film, or standing water. Replace chairs, trash cans and other items to their original location.

1.5.3. PERIODIC CARPET RESTORATIVE CLEANING. Floors shall be vacuumed prior to performing floor maintenance. Move chairs, trash cans and other easily moveable items to perform the service. Deep clean the carpet using steam cleaning or shampoo to address soil suspension, soil extraction and drying. Ensure after cleaning that the carpeted area shows an improvement in visible appearance. Remove a cleaning solution residue from baseboards, furniture, trash cans, chairs and other similar items. Move chairs, trash cans and other moveable items back to their original location.

1.5.4. LOW DUSTING (LEVEL 4 ONLY). Low dusting includes surfaces within 6 feet for the floor. Surfaces must be free of dust lint, cobwebs and litter. Surfaces could include tables, shelves, bookcases, window sills, handrails, window blinds, etc.

1.5.5. HIGH DUSTING (LEVEL 4 ONLY). High dusting includes surfaces above 6 feet and up to 10 feet. Surfaces must be free of dust, lint and cobwebs. Surfaces could include storage lockers, window sills, clocks, pictures, window blinds, ceiling fans, air vents, etc.

1.5.6. CLEAN INTERIOR AND EXTERIOR WINDOWS, GLASS AND MIRRORS (LEVEL 4 ONLY).

Clean all windows, glass and mirrors (not in restrooms). Glass includes glass in doors, partitions and walls. This category may be broken down into separate tasks. Glass shall show no traces of film, dirt, smudges or other foreign matter.

1.6. EMERGENCY CLEANING SERVICES.

1.6.1. ONE-TIME EMERGENCY CLEANING. Emergency cleaning services may be required to be performed in the event of a confirmed COVID-19 or other health epidemic/pandemic case in a facility. These services shall be requested by the COR on a work order by building square feet.

1.6.1.1. The emergency deep cleaning service provides a one-time cleaning and disinfecting of all “frequently touched surfaces” in the facility, including those surfaces in office areas, common areas, break areas, hallways, corridors, stairs, etc. Cleaning shall be performed with products and solutions outlined in paragraph 1.1 and sub paragraphs and/or those recommended by the Centers for Disease Control and Prevention. All new hazardous cleaning products must be approved IAW section 4.4. – Environmental Requirements.

1.6.1.2. Frequently touched surfaces include, but are not limited to, the following:

- All stairwell railings

- All elevator cab railings and buttons

- All drinking/bottle filling fountain – all surfaces

- All light switches and switch plates

- All door handles or door knobs of interior and exterior doors, including stairwell access doors

- In common areas and break areas - Counters, sinks, paper product fixtures, trash can covers

- In restrooms – counter, sinks, sink and shower fixtures, paper products fixtures, trash can covers, toilets

- Other frequently touched surfaces

1.6.2. HEALTH CRISIS FACILITY RESTROOM CLEANING. If the installation Health Protection Condition (HPCON) levels warrant increased restroom cleaning or if there is a confirmed case of COVID-19 or other health epidemic/pandemic case in a facility, emergency daily cleaning of restrooms may go into effect for Level 1 and Level 2 facilities. Restrooms shall be cleaned 5 days a week (Monday through Friday) for one month unless otherwise directed by base Public Health. Level 3 and Level 4 facilities already received daily cleaning so no increase is warranted for these facilities.

1.6.3. The Contractor shall follow CDC guidance for Personal Protective Equipment (PPE), cleaning methods and cleaning products to accomplish these tasks.

1.6.4. Emergency one-time deep cleaning shall be a priority over other regular cleaning tasks to mitigate any community spread. The Contractor shall schedule cleaning to be completed within 24 hours of work order issuance. Additional time should be afforded for buildings with more than one floor or larger buildings.

1.7. CDC AND YOUTH CENTER SPECIAL CLEARANCE REQUIREMENTS.

Special clearance requirements exist for the Contractor personnel to work at the Youth Center and Child Development Center (CDC). The COR suggests that 25% of the contractor personnel have favorable background checks to be able to perform the services at the Youth Center and CDC, if required. Lack of personnel with access DOES NOT relieve the Contractor from performing the custodial services at these facilities and will result in a deficiency write-up for every service missed at these facilities. Personnel working at the CDC and Youth Center shall be required to have a CAC card. See section 4.5.2. for the process.

1.7.1. CRIMINAL HISTORY BACKGROUND CHECK. (Reference DoDI 1402.5, E.6.1.2 and AFI 34-144, 10.6) In accordance with DoDI 1402.5 Criminal History Background Checks on Individuals in Child Care Services, required background checks must be initiated and completed for Contractor personnel before being allowed to work in the CDC and/or Youth Center. All background check results shall be maintained by the CDC Director. Required forms shall be emailed to the site manager for distribution. Forms must be filled out electronically, except for signatures. The following steps are required to process the background checks required.

1.7.2. INSTALLATION RECORDS CHECK (IRC). Contractor personnel are required to fill out Sections I and II on a DD Form 3058 or a local form provided by Security Forces. This form shall be provided to the Contractor Personnel by the COR. Personnel shall email the completed form to the CES Security Manager to initiate an IRC. The IRC must be processed and completed before the Contractor personnel are authorized to work in the CDC and/or Youth Center.

1.7.3. NATIONAL AGENCY CHECK WITH INQUIRIES (NACI). Contractor personnel are required to fill out Section I on an AF Form 2583 to initiate this request. This form shall be provided by the COR. Personnel shall email the completed form to the CES Security Manager to initiate an NACI. Once the AF 2583 is processed into the system, the personnel shall receive an email to complete an E-QIP application on line. This is required to complete your NACI.

1.7.4. FBI FINGERPRINT CHECK AND CRIMINAL HISTORY SUMMARY. The Contractor shall fill out the FBI Fingerprint application and payment requirements included in Exhibit 1 of the contract. The contract personnel shall call 481-6783 or 481-7455 to schedule fingerprinting at the Information Protection Office in building 72. Once these steps are completed, the contract personnel shall take all paperwork (including the fingerprint card, money order/certified check or credit payment information) to the COR Office in building 358 to be mailed to the FBI. If paperwork is not complete, the contract personnel must correct the form(s) before the package can be sent to the FBI. FBI Fingerprint result will be mailed directly to Security Forces who will forward results to the FSS Security Manager. Estimated turn around for the FBI Fingerprint process is 6 weeks.

1.7.5. BACKGROUND CHECK RESULTS. All of the above required documents must be submitted and the Security Manager shall identify all report results whether favorable, unfavorable or derogatory to the Contracting Officer (CO). An unfavorable or derogatory report would include, but not be limited to, the following applicable crimes: any charge or conviction for a sex crime, an offense involving a child victim, a substance abuse felony or a violent crime.

The CO shall provide the appropriate documentation for the CDC Director’s file. No contract personnel shall be allowed to work at the CDC or Youth Center with an unfavorable or derogatory IRC and/or FBI fingerprint report.

1.7.6. EMPLOYEE STATEMENT. All Contract personnel assigned to the CDC and/or Youth Center shall sign an Employee Statement form indicating whether or not they have been arrested or convicted of any crime involving children, drugs or alcohol (Appendix D). This document shall be turned in to the CDC director.

1.7.7. RE-VERIFICATION OF BACKGROUND CHECKS. Within 2 months of the 5 year anniversary of FBI Fingerprint check, Contractor personnel shall report to the CES Security Manager to initiate a re-verification of the IRC and have a Defense Central Index of Investigations (DCII) initiated. This must be accomplished every 5 years. Contractor personnel shall follow the steps in Section 4.5.3.8. to re-verify the FBI Fingerprint check.

1.7.8. RIGHT TO BACKGROUND CHECK COPY. Contractor personnel have the right to obtain a copy of the background check report from the CES Security Manager. Contractor personnel have the right to challenge the accuracy and completeness of any information contained in the report through the Privacy Program described in DoDI 5400.11-R. The CO shall inform the Contractor of personnel whose criminal history background checks result in an unfavorable or derogatory report. The Contractor shall immediately remove these personnel from their service at the CDC or Youth Center.

1.7.9. PROOF OF BACKGROUND CHECKS. Contractor personnel who have previously received an IRC and FBI Fingerprint background check must provide proof of the check to the CES Security Manager who will determine if and when a new background check is required.

1.8. CDC AND YOUTH CENTER DOCUMENTATION REQUIREMENTS. All

contract employees that work at the Youth Center and/or CDC are required to be up-to-date on immunizations and provide written documentation.

1.8.1. REQUIRED IMMUNIZATIONS FOR CDC AND YOUTH CENTER. All contractor personnel assigned to the CDC and/or Youth Center shall be free of communicable diseases, to include tuberculosis, prior to entering either facility. All Contractor personnel must have the following immunizations required by AFJI-48-110, “Immunizations and Chemoprophylaxis,” prior to personnel being allowed to work. The Contractor shall be responsible for providing certification that all of his/her personnel meet the above conditions. Payment for any required immunizations will be incurred at the Contractor’s expense.

1.8.2. REQUIRED DOCUMENTATION. Submit a summary listing of all personnel proposed to work in CDC and/or Youth Center to the CO and COR prior to start of work by any new personnel. The CO shall forward a copy of the listing to the CDC Director any time it is updated. The listing shall include the personnel’s name, position, date background checks were initiated, date child abuse training was completed, certification that immunization requirements have been met and the Employee Statement for assignment to work in the CDC and/or Youth Center. In the event that Contractor personnel are replaced, even temporarily, the replacement must meet all requirements set forth in Section 1.7. prior to beginning work.

1.8.2.1. Failure to fulfill these requirements, in sufficient time to replace Contractor personnel, will not relieve the Contractor of their responsibilities under this contract. Failure to provide these statements will prohibit Contractor personnel entrance into the CDC and/or Youth Center.

1.8.2.2. Update the spreadsheet as necessary as personnel are changed or added to work at the CDC and Youth Center. Submit a summary health report for contractor personnel from the Health Department or their doctor and current immunization records to the CDC Director prior to Contractor personnel starting work and on an annual basis.

1.9. CDC AND YOUTH CENTER CLEANING PRODUCTS. The Contractor shall provide a list of all approved cleaning products to be used in CDC and Youth Center to the CDC Director. The CDC Directors shall ensure that the cleaning products are suitable for use in a child care environment. Cleaning products containing warnings that they may not be used in areas occupied by children CANNOT be used. Chemical air fresheners are not permitted.

1.9.1. SOLUTION STORAGE. Store all disinfecting, cleaning, and sanitizing solutions in their original, labeled containers (with the exception of bleach water solution). Store bleach water solution in a non-brand name container clearly labeled as “BLEACH WATER.” Use the same type of container for bleach water throughout the facility. Keep all cleaning solutions out of children’s reach at all times.

1.9.2. CLEANING EQUIPMENT/MATERIALS. The Contactor shall have use of Janitor closets in facilities where space is available. The Contractor is responsible to ensure that equipment, trash cans, mops, brooms and cleaning buckets are rinsed/cleaned and are free of odor and dirt.

Cleaning equipment shall be inaccessible to children.

1.10. SPECIAL CLEARANCE FOR ACCESS TO FACILITIES WITH MAGNETIC

LOCK SYSTEMS. Facilities with magnetic lock systems require a Common Access Card (CAC) to gain access to the facility. The CAC is a controlled item and may not be used outside of performance of this PWS. Required forms shall be emailed to the site manager for distribution. The steps for obtaining a background check and CAC are located in Section 4.5.

1.10.1. The Contractor is responsible to notify the COR IMMEDIATELY upon personnel termination and collect ALL CAC cards for personnel no longer working at these buildings or for the Contractor. The CAC cards shall be turned in to the COR.

1.11. CUSTODIAL SERVICES EXTENDED HOURS OF OPERATION AND SPECIAL

RESTRICTIONS. The facilities listed in the table below have extended hours of operations or special service performance restrictions.

1.11.1. If for any reason the occupants of a building prevent the Contractor from providing scheduled services, the Government will forfeit that cleaning. The Contractor shall notify the COR in writing with date, building number, specific areas affected and NAME of person refusing service.

1.12. CONTRACTOR PROVIDED EQUIPMENT AND SUPPLIES.

1.12.1. CONTRACTOR-FURNISHED EQUIPMENT/VEHICLES. Provide and maintain all equipment and vehicles necessary to perform the requirements of this contract. The Contractor may perform cleaning, minor repair and other normal maintenance requirements of custodial equipment on site. Maintenance and repair of Contractor vehicles or their privately owned vehicles shall NOT be accomplished on site. Contractor vehicles shall prominently display and maintain company name on both sides of vehicle to present a neat, professional appearance.

Contractor vehicles shall be used solely for the intended purpose of performing services in this PWS during normal hours of operation.

1.12.2. EQUIPMENT CONDITION. All equipment used in performance of this contract shall carry an Underwriters Laboratory (U/L) listing and be maintained in good operable condition.

Equipment found to be unsafe and/or unable to function as designed, shall not be used in performance of this contract. The Contracting Officer (CO) or COR may inspect the Contractor’s equipment and vehicles at any time and direct the removal of any unsafe or unusable equipment or vehicle from the installation.

1.12.3. EQUIPMENT QUANTITIES. The Contractor shall provide an adequate number of equipment items and vehicles to effectively fulfill the scope of this contract. In addition, the Contractor shall maintain or have backup capability to provide continued service in the event primary equipment or vehicles are down for extended maintenance or repairs. Equipment failure does not alleviate the Contractor from performing any requirement contained in this contract.

1.12.4. CONTRACTOR FURNISHED SUPPLIES. One hundred percent of all paper products procured (toilet tissue and paper towels) shall contain recycled materials.

- Plastic Trash Can Liners/Bags: Suitable for the type and size of trash receptacle.

- Toilet Tissue: Two-ply

- Paper Towels: C-Fold and Tri-Fold paper towels (9-3/16” or 10-18” widths)

- Hand Soap: Liquid anti-bacterial soap for installed liquid soap dispensers or hand size Push/pump liquid anti-bacterial soap when installed dispensers are absent or non-operational

NOTE: Contractor is required to provide enough stock to last through the next scheduled service. Calls for re-stocking supplies will be a contract write-up.

1.13. MISCELLANEOUS REQUIREMENTS. During and after performance of services, the Contractor shall:

- Turn off unnecessary lights

- Report fire and safety hazards

- Report items (such as toilets) in need of repair to the facility manager or COR

- Turn in lost and found articles to the facility manager or COR

- Notify the COR the next morning if areas were occupied and unable to be cleaned during scheduled time

- Notify Security Forces/COR when unauthorized or suspicious persons are seen on premises

- When entering any facility or area after it has been secured, enter through one door and lock the door behind them.
