Attachment 1.xlsx - Find Your Future Portal RFP Response Template

This file is a contractor information template for the State of South Carolina's Find Your Future Portal procurement, requiring respondents to provide comprehensive documentation of their expertise, experience, and operational capabilities in implementing citizen experience, digital government, and multi-agency portal solutions. The template mandates complete responses across all sections including company background, minimum qualifications, references, subcontractors, tools and accelerators, and RFP exceptions. Vendors must demonstrate at least fifteen years of continuous operation, ten years of experience delivering enterprise-scale, user-facing digital platforms with multi-agency integrations, and successful implementation of at least two state or federal government contracts within the past ten years. Respondents must provide a minimum of two references from similar public sector implementations in North America, with the option to submit up to five total references demonstrating comparable scope, industry, and complexity. The template requires disclosure of any contract terminations for cause in the past twenty-four months, lawsuits within the preceding five years, and bankruptcy filings within the past decade.

The RFP response template requires vendors to detail their organizational structure, including total US-based clients served, annual implementation services revenue for the past three fiscal years, and year-over-year growth metrics. Respondents must identify the total number of US-based and global consultants in their public sector practice, locations of regional hubs and centers of excellence, and certifications or partnerships with relevant platforms. The template requests enumeration of completed and ongoing similar projects for multi-agency public sector clients, with specific focus on large-scale state or federal agency implementations. Vendors must commit to on-site staffing with a dedicated project manager and implementation team for no fewer than three days per week throughout the implementation and hypercare periods. The template includes sections for documenting subcontractors with detailed information on their legal entity type, ownership structure, employee count, proposed services, and relevant project experience, along with corresponding references. Vendors must also provide an inventory of project tools and accelerators offering differentiated value that are embedded in pricing, and may submit exceptions to RFP materials with rationale and proposed modifications for state consideration.
