About this file

This document is a Past Performance Questionnaire for a federal lease solicitation (RLP 36C24425R0039) issued by the Department of Veterans Affairs Veterans Health Administration. The solicitation seeks to lease up to 9,990 Rentable Square Feet (RSF) in Pittsburgh, PA for an Emergency Response and Engineering Center (EREC), yielding 7,400 Net Usable Square Feet (NUSF). The questionnaire is designed to collect detailed performance evaluations from previous contractors, with assessors rating the potential contractor's performance across seven key areas: quality of service, schedule/timeliness, customer satisfaction, management/personnel, cost/financial management, safety/security, and general performance.

The questionnaire uses a rating scale from Exceptional (E) to Unsatisfactory (U), with specific criteria for evaluating past performance, including building design quality, compliance with schedules, customer interaction, management effectiveness, cost control, and safety adherence. Assessors are requested to complete the questionnaire by September 15, 2025, and submit it electronically to Ashley Saeler at Ashley.Saeler@va.gov. The document emphasizes the importance of comprehensive and timely feedback to support the evaluation process for this lease opportunity.
