About this file
This is a Past Performance Questionnaire template for use in evaluating contractor proposals for various ship repair works on the USS MOUNT WHITNEY. The questionnaire is administered by NAVSUP FLC Sigonella, Naval Regional Contracting Detachment Naples, and must be completed by the offeror's customers rather than the offeror itself. Questionnaires must be returned by the proposal due date; those received after the deadline may be disregarded at the Government's discretion. The vessel specifications indicate a 634-foot-long Navy warship with a gross tonnage of 18,874 tons and a design deadweight of 6,439 tons.
The questionnaire is divided into two sections: Section A requires the offeror to provide contractor information, contract details, period of performance, place of performance, work description, and point of contact information along with confirmation of the method and date the questionnaire was sent to the customer. Section B is completed by the evaluator (customer) and assesses contractor performance across five primary categories using a four-point adjectival rating scale (Outstanding, Acceptable, Unacceptable, Not Applicable): Quality and contract requirements achievement, Timeliness of performance and submissions, Contractor responsiveness to customer concerns and problem resolution, Cost control and adherence to contract pricing, and contract administration. The evaluator also provides an overall assessment of willingness to do business with the contractor again. Questions and answers regarding this solicitation should be directed to Contract Specialists Ms. Annamaria Guarino, Ms. Maria Di Lieto, or Ms. Nicole Barnhart at the provided Navy email addresses.
