About this file

This is an Indian Economic Enterprise (IEE) Representation Form required for submission under the Buy Indian Act for IHS solicitation RFQ-25-PHX-008. The form serves as a self-certification document where offerors must indicate whether they meet the definition of an "Indian Economic Enterprise" with "Indian Ownership" under the Buy Indian Act.

The form requires offerors to maintain IEE eligibility throughout the proposal period, contract award, and full contract performance period, with immediate notification required if eligibility status changes. Required information includes the name of the 51% owner, business name, federally recognized tribal entity, and Unique Entity Identifier (UEI) number. The form emphasizes that false representations are punishable under federal law and requires registration in the System for Award Management (SAM).
