About this file

This document contains a past performance questionnaire for a contractor bidding on a federal contract to construct a shop building for the U.S. Fish and Wildlife Service in Madison, South Dakota. The building is to be a 55 foot by 80 foot pre-engineered metal structure to serve as the maintenance shop for the Madison Wetland Management District, relocating and reusing equipment from the existing shop. The questionnaire evaluates the contractor's performance on previous similar contracts in areas such as personnel management, quality control, problem solving, and compliance. It requests ratings for each category as satisfactory, unsatisfactory, or unknown/not observed along with narrative explanations. The evaluator is asked to provide contact information and assess whether they would award the contractor another contract based on past performance. The solicitation number and point of contact are provided to aid in tracking responses received for a contractor's past performance evaluation for the shop building construction project.
