The contractor shall remove wire from existing failed conduits running from Launch Support Buildings (LSBs) to power poles at all five sites (approximately 103 linear feet each) and from LSBs to gate switches at H-09 and I-04 sites (approximately 70 linear feet each). The contractor must cut and cap existing conduits a minimum of 12 inches below grade and abandon in place. New 1½-inch PVC-coated rigid conduit will be installed at a minimum depth of 36 inches from the LSBs to power poles and gate switches, with new 10/3 copper stranded wire pulled through and properly terminated. The contractor shall separate the top 6 inches of pad and access road gravel for reuse as backfill or provide equivalent new material. Backfill must be placed in maximum 6-inch loose lifts with mechanical compaction, an electrical warning tape placed 18 inches above the conduit, and native materials used to restore surface conditions. The five work sites are located in Colorado and Nebraska at distances of 90 to 127 miles from F.E. Warren AFB. The Period of Performance is 120 calendar days, with work hours between 7:30 a.m. and 4:00 p.m., excluding weekends and federal holidays. The contractor must provide weekly project updates, comply with extensive security requirements including Real ID-compliant identification for personnel and Entry Authority Lists for site access, obtain AF Form 103 Dig Permits 30 days prior to excavation, coordinate hazardous materials with Base Environmental, maintain comprehensive safety programs, and submit all material submittals for approval via DoD SAFE prior to ordering. Gate switches must be replaced with Leviton 1221-21 Single-Pole 20A 277Volt Ivory switches, with existing switches returned to 90 CEM. The contractor is responsible for all utilities, equipment, licenses, permits, insurance coordination, and site cleanup, with work conducted in compliance with National Electrical Codes and applicable federal, state, and local regulations.

Statement of Work

OVERVIEW

1. This project includes replacing site lighting electrical conduits at five (5) Launch Facilities (LFs). The LFs are under the control of F.E. Warren Air Force Base, Wyoming. These specifications describe the components and requirements for the work under this contract.

2. The Period of Performance (PoP) for this project shall be 120 calendar days.

3. The work shall be performed at MAFs/LFs located in Colorado and Nebraska. The distances from F.E.

Warren AFB are indicated below.

SCOPE OF WORK

1. The Contractor shall provide all labor, materials, equipment, and transportation necessary for the site lighting electrical conduits at five (5) Launch Facilities (LFs) as specified in this Statement of Work, in strict compliance with the specifications, applicable drawings, attachments, and the Terms and

Conditions of the Contract as well as all applicable building codes, standards, and practices in the industry. The period of performance (PoP) for this contract will be 120 calendar days.

a. The contractor shall remove wire from existing failed conduit, LSB to power pole at all five sites. In addition, on H-09 and I-04 remove wire from existing failed conduit, between the LSB and the gate switch.

b. The contractor shall cut and cap each end of existing conduit a minimum of 12” below grade and abandon in place.

c. The contractor shall install new one-and-one-half inch (1 1/2“) PVC coated rigid conduit from the LSB to the power pole at each site a minimum of thirty-six inches (36”) deep. In addition, on sites H-09 and I-04 install new 1 1/2“ PVC coated rigid conduit a minimum of 36 inches deep from the LSB to the gate switch.

d. The contractor shall separate top 6” of pad and access road gravel to be reused as the top of backfill or provide like gravel pad and access road materials for the top 6” of excavation backfill.

i. The contractor shall excavate in a manner so that no adjacent utilities are damaged.

e. The contractor shall pull new wire through the new conduit and properly connect circuit to ensure proper operation of the site light system.

f. The contractor shall backfill conduit trench in maximum of six-inch (6”) loose lifts, apply compactive effort until soil is firm and tight when probed with a steel tee handled probe to ensure there will be no backfill settlement under the finished pad.

g. The contractor shall use a mechanical apparatus designed and intended to compact soil to compact the backfill material.

h. The contractor shall place an electrical warning tape eighteen inches (18”) above conduit during backfilling efforts.

i. The contractor shall finish backfill of trench with the separated soil materials to match the existing materials used for the pad.

j. The contractor shall purchase new materials to match existing pad material if the CO and 90

CES/CEM determine that the separated materials have become contaminated.

k. The contractor shall provide submittals of manufactured items proposed for this project to 90

CES/CEM prior to ordering manufactured items.

l. The Contractor shall provide weekly project updates to 90 CES/CEM upon award of this project.

m. The Contractor shall hold project meetings as needed to coordinate project activities with CO and 90 CES/CEM at the request of the CO or 90 CES/CEM.

n. The Contractor shall include in the proposal the below for technical review. Failure to include any item may result in a proposal being determined to be technically deficient.

o. Lead time for materials to be purchased and delivered to the site.

p. Contractor certifications or licenses for electrical work.

Equipment proposed for earthwork.

2. The Contractor is responsible for supplying and installing a turnkey, fully functional, tested, and operational product meeting the performance requirements outlined by these specifications. The

Contractor will provide any special permits or licenses that may be required, obtain adequate insurance, and coordinate all inspections required by applicable laws and regulations, except as otherwise specified herein.

3. The Contractor will keep a copy of the most current Statement of Work and drawings on the jobsite.

4. Accomplish all work in accordance with the Contract specifications.

5. The Project Coordinator shall be the Contracting Officer (CO) from the 90 Contracting Squadron, and the Project Manager shall be the 90 Civil Engineer Squadron/Missile Engineering Flight (90

CES/CEM).

6. In the case of the winter season that has direct impact to performing the work outlined in this SOW, the

Contractor shall notify the Government they can no longer perform work. 90 CES/CEM will validate that the weather no longer allows work to be performed within the specifications.

a. If the reason for stopping work is due to weather impacted material supplies, the Contractor shall provide a letter to support the winter suspension.

b. The project will be placed into suspension and the remaining number of PoP days will be documented.

c. When weather conditions become favorable for work, the CO will begin coordinating with the contractor to establish a timeline for their return to work.

d. When the suspension is lifted, a new completion date will be established based on the remaining PoP days.

SECURITY REQUIREMENTS

1. The Contractor and their personnel, to include employees and any subcontractors, shall comply with pertinent Air Force security regulations while working on the project. Special security clearances such as Confidential, Secret, etc., are not required of the Contractor or Contractor representatives for the performance of this project.

2. Real-ID Compliance Identification: IAW DodM 5200.08V2_DAFMAN31-101V3_DAFGM 2024- 01, Installation Perimeter Access Control.

a. Base access and Entry Authority Lists (EAL) requests will require Real ID-Compliant identification for base access and EAL creation.

b. Failure of the vendor to ensure personnel have Real ID-compliant identification will result in denial of base access and EAL requests.

c. Levels of Security: Individuals performing work under this contract shall comply with applicable program security requirements. Contractors will support the following levels of security: Unclassified and Unclassified but Sensitive.

3. Base Access: The Contractor shall request access onto F.E. Warren AFB through the CO or authorized representative a minimum of three (3) working days prior to needed entry.

a. One form of ID, as a legible photocopy or electronically scanned valid driver’s license, state photo ID card, or official state/federal photo ID card shall be submitted for each employee requiring base access.

4. Missile Field Access/EALs: Access to MAF/LF is controlled by the U.S. Air Force. All Contractor personnel, vehicles, and equipment entering the site shall be listed on an effective Entry Authority List (EAL), created by the Air Force.

a. The Contractor shall provide, via DoD SAFE, complete, valid, and legible data to be used to produce the initial EAL, prior to their initial commencement of work, and to maintain the EAL.

The standard processing time for all EAL actions is thirty (30) calendar days. Any request to create add to, modify, correct, or renew the EAL shall be submitted at least thirty-one (31) calendar days before needing entry to site. The Contractor shall remain current on information that may change or expire (e.g., IDs or license plates) during the contract period and shall submit updates accordingly. Any delays caused by the submission of incomplete, invalid, or illegible data will be at no cost to the Government. 90 CES/CEM will provide the expiration date for each EAL, when the date is made effective, to the Contractor and the CO.

b. The Contractor shall submit the following for each employee (to include prime, subcontractor, and delivery/testing services employees).

i. One form of ID, as a legible photocopy or electronically scanned valid driver’s license, state photo ID card, or official state/federal photo ID card. The same ID shall be shown for entrance onto site once the EAL is effective.

c. The Contractor shall submit the following for each vehicle and piece of equipment, to include trailers. Standard hand-held equipment is not required to be on an EAL.

d. The Contractor shall submit the following for each camera.

5. Site Entry:

a. To gain entry onto site, Contractor personnel shall be

i. Listed on an effective EAL.

ii. Identified by Escort Official.

iii. Able to produce, for verification, the exact form of ID given for the EAL process.

iv. Submit to physical and electronic search of person and materials.

v. Physically escorted on-site by an authorized Escort Official and remain under constant escort surveillance while on site.

vi. Not in possession of any banned electronic devices.

vii. In possession of equipment/cameras listed on an effective EAL.

b. Upon arrival, the Escort Official will identify all Contractor personnel and verify identifications.

c. Contractor personnel not listed on the EAL or those who cannot provide proper identification shall not be permitted on site.

6. Vehicles/Equipment:

a. Contractor vehicles and equipment listed on an effective EAL will be allowed to park inside the security fence to support the work under this contract and shall be searched by Security personnel prior to entry.

i. The Contractor shall obtain approval from 90 CES/CEM prior to leaving vehicles and equipment parked overnight.

Overnight parking inside the security fence at the LFs will not be allowed.

ii. Contractor vehicles and equipment shall be placed so as not to interfere with gates, emergency escape routes, antenna, and other above ground structures.

b. The access road from the main road to site is controlled by the U.S. Air Force. The area adjacent to the access road is private, federal, state, or county property.

i. The Contractor shall obtain approval from 90 CES/CEM prior to parking vehicles and equipment on the access road.

Vehicles parked on the access road shall be clear of the perimeter fence or gate by a minimum of twenty-five feet (25’).

7. Contraband:

a. The Contractor shall ensure contraband items are not brought onto site by Contractor personnel.

The following items are considered contraband on MAF/LF sites.

i. Privately owned firearms to include rifles, shotguns, handguns, air/pellet guns, and sling shots.

ii. Privately owned ammunition or unauthorized ammunition.

iii. Knives less than four inches in length are permitted.

iv. Explosives, except for official use or storage.

v. Illegal drugs or controlled substances.

vi. Equipment/cameras not listed on an effective EAL.

b. Cellular phones are permitted in certain areas; however, the use of photographic capabilities is never permitted. The Escort Official and/or Security personnel will identify areas where cellular phones are/are not permitted and will confiscate improperly used cellular phones.

8. Anti-Terrorism:

a. Notifications: If the Contractor (contract employees/subcontractors) identify any suspicious activity, they shall contact the Base Defense Operations Center (BDOC) at 307-773-3501 (on-base)/Missile Security Control (MSC) at 307-773-2701 (off-base) and notify the CO and 90 CES/CEM that they contacted the BDOC and/or MSC.

b. Training: Contractor employees that require routine physical access to a Federally-controlled facility or military installation shall complete Level I antiterrorism awareness training within thirty (30) calendar days of requiring access and annually thereafter and send a copy of their employees’ training certificate(s) to the CO. 9. F.E. Warren AFB Communications: The majority of work will be at areas where commercial phone service is not available. The following phone numbers are provided for assistance in conducting on-base telephone communications.

10. Interrupted Site Access: If existing access to the site is to be temporarily blocked, temporary access shall be properly provided by the Contractor. The Contractor shall notify the CO twenty-one (21) https://jkodirect.jten.mil/Atlas2/page/login/Login.jsf calendar days prior to any interruption of access to the site. The date, site(s) affected, length of time, and alternate entry method shall be submitted on a Site Interruption Plan for approval.

11. Vacating The Work Area: During exercises, natural disaster, and national emergencies or for security reasons, the Contractor may be required to vacate a work area.

12. Staging/Storing: Materials and equipment not stored at Contractor’s facilities shall be stored on approved Air Force property or approved adjacent private or public properties.

a. Staging/Storing on Air Force Property:

i. The Contractor shall obtain approval from 90 CES/CEM prior to storing on Air Force property. Unkempt, insecure, or unsightly storage shall not be allowed.

There is limited facility storage space at the MAFs and shall only be allowed on a case-by-case basis.

There is no facility storage space at the LFs.

ii. A storage plan may be required if storage containers are to be left on or near the site, for security considerations.

b. Staging/Storing on Adjacent Properties:

i. Contractors not storing on Air Force property shall store on adjacent private or public properties within the Limit of Potential Ground Disturbance as shown in Attachment 1.

ii. The Contractor shall obtain written consent of the landowner or leaseholder.

13. Adjacent, Private, &/or Public Property/Landowners/Leaseholders: Contractor shall obtain written agreements and lien releases with adjacent private and public landowners and leaseholders when operations require use of these properties for storage or access to Air Force property. Landowner Agreements will allow the use of property by Contractor and the lien releases shall release the Air Force from any damage to private property because of Contractor operations. Where leaseholders are present on the property, the signatures of both landowners and leaseholders are required. Copies of agreements and lien releases shall be supplied to the CO prior to the start of work on each site. The Contractor will not be allowed to start work on a site until agreements are in place.

14. Operational Security (OPSEC): The Contractor shall adhere to the following minimum requirements in support of this requirement:

a. All contractors, subcontractors, their employees, associates, and consultants who have access to critical information and indicators shall comply with the Air Force-sponsored Operations Security (OPSEC) program.

b. OPSEC is an information-related capability that preserves friendly essential secrecy by using a process to identify, control, and protect critical information and indicators. If critical information and indicators are disclosed, it could allow adversaries or potential adversaries to identify and exploit friendly vulnerabilities leading to increased risk to mission failure or the loss of life. The desired effect of OPSEC is to influence the adversary’s behavior and actions by reducing the adversary’s ability to collect and exploit critical information and indicators about friendly activities.

c. Responsibilities:

i. The Contractor shall ensure employees receive OPSEC education that includes an overview of the OPSEC process; the definition of OPSEC; its purpose; what is critical information; the individual’s role in protecting critical information; and the general adversary threat to the AF’s critical information. Additionally, OPSEC education shall include updated threat information, changes to critical information and indicators, new procedures and/or OPSEC measures implemented, altered, or deleted by the organization.

ii. DoD Contractors with OPSEC requirements or those who have access to critical information and indictors shall complete initial and annually-recurring refresher training.

Accomplishment of initial OPSEC training shall not exceed ninety (90) calendar days from initial assignment to a contract with OPSEC requirements. Access to mission-critical information will not be allowed until OPSEC education has been completed and documented.

iii. OPSEC education is a continuous requirement. The host unit OPSEC coordinator shall track OPSEC training statics to include the personnel working on the contract, when each individual has received the required initial OPSEC training, and when refresher training is due and/or completed.

iv. The Contractor’s personnel shall not discuss government operations in public or over unprotected or unencrypted communications. Controlled unclassified information (CUI) may only be transmitted as directed in this SOW.

v. The Contractor shall not post to company websites, publications, newsletters, or any other media any images, data, or any other information that reveals sensitive government operations, personnel, equipment, and/or classified or controlled unclassified information. When in doubt, the company press release related to this contract should be coordinated through the COR or CO, as applicable.

vi. Prior to seeking approval through F.E. Warren AFB Public Affairs, any information to be released outside of official U.S. Government channels, via any media to include electronic communication, shall be approved through an OPSEC review. All personnel shall promptly report OPSEC-related incidents, issues, and concerns to the host-unit OPSEC coordinator.

vii. The Contractor shall restrict access to those individuals with a valid need-to-know who are actually providing services under the contract. Further dissemination to other contractors, subcontractors, or any other government agencies and private individuals or organizations is prohibited unless authorized in writing.

viii. Because observation of events, operations, physical changes, etc. may reveal National Security Information, specific restrictions are needed to preclude unintentional release of this information to unauthorized parties. Unauthorized disclosure and transfer of National Security Information is punishable under 18 USC§793. Therefore, the Contract Personnel shall not disclose to unauthorized third parties, post to unofficial sites to include Social Networking sites any images, data/information, or observed events that reveal sensitive government operations, personnel, equipment, including but not limited to Tactics, techniques, and procedures, production, work schedules, any visible or concealed modifications, upgrades, additions to vessels, aircraft, or weapons or equipment; increases, change, or decrease in work/deployment frequency or government personnel, vehicles, vessel, and/or aircraft movements; specialized equipment orders, deliveries, shipments, etc. Unauthorized disclosures and attempts to solicit this type of information by unauthorized third parties or others not affiliated with this contract shall be reported to the COR or CO, as applicable.

Government issued badges, identification shall be removed and/or concealed from plain sight when off base and shall not be left in vehicles or unprotected areas.

Badges and passes may not be duplicated, copied, or loaned to others. Lost or stolen identification badges, vehicle passes, etc., will be immediately reported to the COR, CO, and/or installation Security Office.

15. Controlled Unclassified Information (CUI):

a. Contractors shall follow CUI safeguarding and dissemination controls in accordance with Part

2002 of Title 32 CFR DoDI 5200.48, Controlled Unclassified Information; and DoDI 5200.48_DAFI 16-1403, Controlled Unclassified Information.

i. Unlike classified information, an individual or organization generally does not need to demonstrate a need-to-know to access CUI, unless required by a law, regulation, or government-wide policy, but shall have a lawful government purpose for such access.

b. Marking Requirements: Contractors shall follow CUI marking requirements in accordance with DoDI 5200.48 and DoDI 5200.48_DAFI 16-1403.

c. Dissemination: Contractors shall follow CUI dissemination requirements in accordance with DoDI 5200.48 and DoDI 5200.48_DAFI 16-1403.

d. Storage: Contractors shall follow CUI storage requirements in accordance with DoDI 5200.48 and DoDI 5200.48_DAFI 16-1403.

i. During work hours, steps shall be taken to minimize the risk of access by unauthorized personnel, such as not reading, discussing, or leaving CUI information unattended where unauthorized personnel are present.

ii. After working hours, CUI information will be stored in unlocked containers, desks, or cabinets if the government or government-contract building provides security for continuous monitoring access. If the building security is not provided, the information shall be stored in locked desks, file cabinets, bookcases, locked rooms, or similarly secured areas.

e. Transmission: Contractors shall follow CUI transmission requirements in accordance with DoDI 5200.48 and DoDI 5200.48_DAFI 16-1403.

i. DoD SAFE shall be the primary method of submitting information and documentation.

f. Disposition: Contractors shall follow CUI disposition requirements in accordance with DoDI

5200.48 and DoDI 5200.48_DAFI 16-1403.

i. Record and non-record copies of CUI documents will be disposed of in accordance with

Chapter 33 of title 44, U.S.C. and the DoD Components’ records management directives.

When destroying CUI, including in an electronic form, agencies shall do so in a manner making it unreadable, indecipherable, and irrecoverable. If the law, regulation, or government-wide policy specifies a method of destruction, agencies shall use the method prescribed.

ii. Record and non-record copies of CUI documents may be destroyed by means approved for destroying classified information or by any other means making it unreadable, indecipherable, and irrecoverable the original information such as those identified in NIST SP 800-88 and in accordance with Section 2002.14 of Title 32, CFR.

g. Disclosure: Contractors shall follow CUI disclosure requirements in accordance with DoDI

5200.48 and DoDI 5200.48_DAFI 16-1403.

i. Contractors shall follow CUI disclosure requirements in in accordance with DoDI

5200.48 and DoDI 5200.48_DAFI 16-1403.

PRINCIPLE FEATURES OF WORK PER CSI DIVISION

NOTE: Refer to this Statement of Work, drawings, and attachments for complete details of work. This scope is a general overview of intended work.

CSI 01 – General Requirements

1. Project Scheduling: The Contractor shall submit project schedule to the CO within fifteen (15) working days after award of the Contract.

a. The Contractor shall coordinate all work with the CO and 90 CES/CEM to verify that work can be performed as scheduled before traveling to the work site.

i. Provide a minimum of ten (10) working days’ notice, in writing, before beginning work.

ii. Provide a means for contact after normal working hours for notification of emergency changes to the schedule.

iii. If work is not coordinated as stated, Air Force personnel may not be available, work may not be permitted, and the Government shall not incur any additional cost as a result.

b. Working hours will be between the hours of 7:30 a.m. (start processing on site) and 4:00 p.m.

(terminated work off site) excluding Saturdays, Sundays, and Federal Holidays. An Escort Official (i.e. 90 CES/CEM Inspector or other) will be provided during these work hours.

Processing onto and off site may require approximately thirty (30) minutes each and shall not be considered delay time.

i. Recognized Federal Holidays include:

New Year's Day (January 1).

Birthday of Martin Luther King, Jr. (third Monday in January).

Washington's Birthday (third Monday in February).

Memorial Day (last Monday in May).

Juneteenth National Independence Day (June 19).

Independence Day (July 4).

Labor Day (first Monday in September).

Columbus Day (second Monday in October).

Veterans Day (November 11).

Thanksgiving Day (fourth Thursday in November).

Christmas Day (December 25).

c. AF Form 3064 Contract Progress Schedule and AF Form 3065 Contract Progress Report shall be submitted monthly, as required.

2. Submittals: Prior to starting any work and in accordance with AF Form 66 Schedule of Material

Submittals (aka “Submittal Register”), submit documentation (e.g., EAL information, plans, agreements, specifications, SDS, etc.) on AF Form 3000 Material Approval Submittal to the CO via

DoD SAFE in accordance with the requirements of these specifications.

a. Submittals shall be provided as one (1) searchable electronic copy via DoD SAFE. Submittals shall be marked with the appropriate classification.

b. Each Contractor-supplied item shall be submitted for approval and approved by the CO prior to installation. In no case shall materials be incorporated into the work unless prior written approval is obtained from the CO.

c. Include outlines, dimensional, assembly and subassembly drawings, parts list and other data required to completely describe each item to be used.

d. When more than one model, series or design is illustrated on any one submittal sheet, the item of intended use shall be highlighted and clearly marked with an arrow or asterisk to eliminate confusion as to the product to be approved. Any materials and/or methods not in strict conformance with the requirements of this contract shall also be submitted for review and approval.

e. All submittal information shall be provided by the Contractor, not less than fifteen (15) working days prior to any site work or by the date determined by the CO at the pre-construction meeting.

A signed and dated copy of the submittal shall be returned to the Contractor after approval or disapproval.

3. Shop Drawings: Shop drawings shall be submitted for all items or components to be fabricated or assembled and requiring detail drawings more than, or to complement, the general drawings and details included in the plans.

a. The plans shall be supplemented by shop drawings as necessary to adequately control the work.

Shop drawings may consist of drawings, diagrams, Safety Data Sheets (SDS), illustrations, schedules, calculations, or other data prepared by the Contractor, subcontractor, manufacturer, supplier, or distributor which illustrate specific portions of the work to be fabricated or installed in accordance with the plan details and specifications.

b. Any work done prior to approval of shop drawings shall be done at the Contractor's own risk.

4. Product Data: Manufacturer’s product data for all commercial products, components, devices, or distinct units shall be submitted by the Contractor for review prior to installation in the work.

a. Product data includes catalog sheets, brochures, SDS, diagrams, schematics, schedules, performance starts, illustrations and any other standard descriptive data.

b. All product data submitted shall be clearly marked to identify pertinent materials, products, or models to be incorporated in the work.

i. All dimensions, clearances, tolerances, performance characteristics, specifications, wiring diagrams, and other such pieces of information shall be clearly marked or highlighted.

5. Certificates of Compliance: A Manufacturer's certificate of compliance for all materials to be incorporated in the work shall be submitted to the CO prior to execution of the work.

a. Certificates shall certify compliance with project specifications, applicable reference standards or specifications, and test data requirements as required.

b. Certificates shall further contain the name, title and signature of the individual certifying the product, and the name and address of the corporation or business supplying the product and certificate.

6. Test Results: The Contractor shall complete testing in accordance with all project specifications, applicable State/Federal/Local rules or regulations, and industry standards or manufacturers’ recommendations and submit results to the CO.

7. Quality Control: The Contractor shall provide and maintain an effective quality control program.

a. The Contractor shall comply with applicable references, standards, specifications, and manufacturers’ recommendations, listed in Appendix 1.

i. The Contractor shall request clarification from the CO if the manufacturers’ instructions conflict with contract documents before proceeding.

8. Utility Services: The Contractor shall provide all utilities, to include but not limited to portable generators, water (drinking and construction), and phone services for Contractor personnel, as needed.

a. When working at the MAFs/LFs, limited utilities are available.

i. At the MAFs, commercial power (120/208V), restrooms, and limited drinking water will be available for use by Contractor personnel.

ii. At the LFs, portable latrines are stationed and will be available for use by Contractor personnel.

b. The Contractor shall provide site facilities for lunch and break areas, as needed.

9. Equipment: Provide all equipment necessary to accomplish the contract work. Non-standard/non-hand-held equipment must be listed on an effective EAL.

a. Power equipment shall be equipped with safety, noise limiting, and fume control devices. The equipment shall be in a safe and efficient operating condition.

b. Only experienced operators shall be permitted to operate the equipment.

c. Plug-in power tools are preferred as wireless power tools are only permitted in certain areas.

d. Cameras are permitted in certain areas if listed on an effective EAL.

10. Excavation: No excavation, regardless of depth, will begin without first obtaining an approved AF

Form 103 Base Civil Engineering Work Clearance Request (aka “Dig Permit”). The Contractor shall submit Dig Permit request(s) to 90 CES Customer Service (307-773-3175) in Building 322 thirty (30) calendar days prior to the planned excavation date. Permits shall be renewed by the Contractor every fourteen (14) calendar days.

a. The Contractor shall provide the following to the Customer Service when planning an excavation:

i. A drawing showing the area to be excavated.

90 CES/CEM can provide an electronic site map to the Contractor for markup.

ii. The purpose of the excavation.

iii. Name, phone number, and organization of the Contractor’s point of contact.

b. The Contractor shall review the permit, keep signed copy on site and shall be responsible for maintaining utility markings during excavation.

c. The Contractor shall notify 90 CES/CEM prior to any excavation.

i. If ground water or landfill debris is encountered, the Contractor shall suspend excavation and notify 90 CES/CEM.

d. Exploratory excavation by hand or vacuum is required within five (5) feet of either side of marked underground utilities.

i. Hand digging or vacuum excavation is required when crossing underground utilities.

ii. If excavation is required parallel and within five (5) feet of an existing utility, exploratory excavation shall be performed by hand or vacuum until the location of the utilities is physically confirmed.

iii. After the location of the utility is physically confirmed, excavation may be performed by machine.

e. Exploratory potholes in roadway areas shall be restored using a flowable fill mixture.

i. In other areas, native backfill material shall be used. The surface condition shall be restored to the pre-existing state.

f. Items encountered and damaged within five (5) feet on either side of a marked line or around a marked items shall be repaired by the Contractor.

i. Items encountered and damaged outside of five (5) feet on either side of a marked line or around a marked point shall be repaired by the Air Force.

ii. In either case, the Contractor shall notify 90 CES/CEM immediately if any damage occurs.

11. Burning/Welding: The Contractor shall obtain an AF Form 592 Hot Work Permit (aka “Burn

Permit”), at least three (3) working days prior to work.

a. Coordinate with the onsite Escort Official or contact 90 CES/CEM at (307) 773-2438 for requirements, initiation, and completion of permit.

b. Contractor shall have the completed permit control number prior to any burning/welding operations.

12. Environmental Compliance: The Contractor is responsible for complying with the State’s or States’ environmental quality regulations, and the Environmental Protection Agency (EPA) Region 7 or 8

Office in Denver for any National Pollutant Discharge Elimination System (NPDES) Construction

General Permits.

a. If a permit is not required by the affected State or Federal agency with jurisdiction on federal sites, the Contractor shall provide written documentation referencing reason for waiver.

13. Green Procurement Program (GPP): In performance of this contract, contractors shall use EPA designated recycled content products, Information Technology (IT) Energy Star products/appliances, Federal Energy Management Program (FEMP) Designated Energy Efficient Low Stand By Power products/appliances, U.S. Department of Agriculture (USDA) Bio based/Bio preferred products, Environmentally preferable products, Electronic Product Environmental Assessment Tool (EPEAT) registered products, Water Sense or other water efficient products, non- or Low Ozone depleting substances under the Significant New Alternatives Policy (SNAP), non or Low toxic or hazardous constituents (e.g. non-VOC paint) and any other environmentally sustainable product/method, to the greatest extent possible.

14. Hazardous Materials: The Contractor shall coordinate all hazardous materials (HAZMAT), to include each compound or chemical intended to be utilized on the site, with 90 CES Environmental

(307-773-2594) for review and approval prior to delivery and use on the site.

a. Hazardous material is defined in Federal Standard, FED/STD-313F (Tables I and II). The definition of materials presenting a health hazard or physical hazard is found in 29 CFR 1910.

b. The HAZMAT approval documentation shall be maintained on the site and all workers shall familiarize themselves with their contents.

c. The Contractor shall be responsible for establishing a system, which tracks the procurement, delivery to site, installation, and disposal of all hazardous materials including proper disposal documents.

d. The Contractor shall be held responsible for proper cleanup and disposal of any hazardous or toxic materials spilled while completing the work in this contract. All cleanups shall be accomplished in accordance with applicable Local, State, and Federal regulations at no cost to the Government.

e. No ozone depleting substances shall be used while completing the work in this contract.

i. This includes the exterior/interior of adjacent buildings and parking lot/access road.

15. Safety: The Contractor is solely responsible for all safety on the project including, but not limited to traffic control, trenching, confined space entry, equipment and materials storage, and compliance by all employees with Base Regulations.

a. Particular care shall be exercised in connection with the operation of vehicles and other equipment on the site.

b. Project safety shall be coordinated with the Air Force prior to commencing service.

c. The Contractor shall notify the CO of all modifications to the Safety Plan.

d. Contractor shall comply with the rules, regulations, standards, and requirements of the

Department of Defense (DoD) AFI 32-7002, Environmental Compliance and Pollution

Prevention, Section 1.3 for Statutes and Regulations (http://www.e-publishing.af.mil ), EPA

40 CFR Subchapter I, Part 260-268 and FED-STD-313C, Tables I and II and 20 CFR 1910, the

Occupational Safety and Health Administration (OSHA www.osha.gov ), and other applicable

Federal, State, and Local requirements regarding disturbing or removing hazardous materials.

e. The federal government is not exempt from compliance with environmental and labor regulations.

f. The Contractor shall notify the CO and 90 CES/CEM of any emergency situation(s).

g. The Contractor shall educate each employee to ensure clear understanding of the following:

i. A review of hazards inherent in the work environment and the precautions required to reduce the hazard.

ii. The Contractor shall always maintain a complete first aid kit.

Location of first aid and other emergency facilities shall be identified.

iii. Detailed description of what to do in case of injury, fire, or identification of a safety hazard.

iv. Review of daily cleanup procedures, including location and review of company safety rules (either posted or handout).

v. The Contractor shall provide all personal protective equipment (PPE) including, but not limited to, ear plugs, goggles, face shields, gloves, safety shoes, and hard-hats. Complete instructions on where and how to use all PPE shall be given by the Contractor.

vi. All personnel shall wear PPE when required by the Contractor or by 90 CES/CEM.

vii. Hospitals near various sites are noted below for emergency purposes.

16. Site Conditions: Daily, or more often as necessary, clean all work areas of debris as well as Contractor tools, equipment, and other materials.

a. This includes the exterior/interior of adjacent buildings and parking lot/access road.

b. The work site shall be returned to a like condition in which it was found prior to start of work or in better condition than commencement of work to the satisfaction of the CO and 90

CES/CEM.

c. Damage to Air Force property caused by Contractor vehicles, operations, equipment, or personnel shall be repaired at the Contractor's expense with no additional cost to the

Government. Skilled craftsmen in the trade shall repair such damage to the satisfaction of the

CO and 90 CES/CEM.

i. Any suspected damage to Air Force property shall be documented and submitted to CO with a detailed description of the type and location of damage within seventy-two (72) hours of damage.

17. Request for Information (RFI): The Contractor shall submit RFIs to the CO via a Request for

Information form prior to any work proceeding.

a. The RFI shall include whether the information or solution requested will be an increase, decrease, or no change in contract cost.

b. Any work done prior to approval of the RFI shall be done at the Contractor's own risk.

18. Modifications: Any change in site conditions or work encountered outside the scope of this contract shall be presented to the CO.

a. A contract modification will be issued by the CO prior to any work proceeding.

19. Red Line Drawings: The Contractor shall submit post-construction (aka “red-lined”) drawings to the

CO prior to project close out.

a. Drawings shall include all changes, revisions, discrepancies, or clarifications encountered during construction.

b. Drawings shall be complete, showing all dimensions, geometry, locations, connections, and operations of elements.

i. The Contractor shall submit the corresponding digital files from electronic drafting software (e.g., AutoCAD or Autodesk), as required.

c. Drawings shall be marked with the appropriate classification.

20. Warranties: Thirty (30) calendar days prior to final acceptance of each delivery order, the Contractor shall obtain and submit written warranties for each equipment, product, good, or service supplied under this contract.

a. Each warranty shall include the name, address, and telephone number of the guarantor’s representative, who will honor the guarantee during the written guarantee period and will provide the service prescribed by the terms of the guarantee.

b. The equipment installed under this contract shall be guaranteed against defective materials, installation, and workmanship for a period of not less than twelve (12) months from the date of acceptance, either for beneficial use or final acceptance, whichever is the earliest, but not before the system has passed all specified tests and been deemed acceptable for operation.

CSI 02 – Existing Conditions

1. The Contractor shall perform an inspection encompassing the existing conditions at each of the five (5)

LF sites and any of the associated components performed during this contract.

a. The Contractor shall verify all quantities, dimensions, and existing site conditions necessary for completion of the project.

2. Existing electrical/communication conduits.

a. Any modifications to/with electrical/communication trench locations due to existing conditions shall be coordinated with 90 CES/CEM.

b. Remove wires from existing conduit to be replaced.

i. LSB to power pole at all five (5) sites (approximately 103 Lineal feet each) and LSB to gate switch at H-09 and I-04 (approximately 70 Lineal feet each).

ii. For conduit to be replaced, cap and terminate existing conduit a minimum of 12 inches below grade and abandon in place.

3. Excavation of new conduit trench

a. Contractor will separate and save the top 6” of gravel materials when crossing the pad or access road to be replaced as the top layer during the backfill procedures or provide 6” of new, “like” gravel materials to be placed as top layer.

b. Trench excavation will be a minimum of 36” deep without any sharp rocks or deleterious materials in the trench.

c. Place 1 ½” PVC coated rigid conduit at bottom of trench, backfill in maximum of 6” lifts and apply significant compactive effort to ensure no settlement of the finished pad or access road.

i. Place an electrical warning tape at 12 – 18” above conduit and continue backfill.

d. Use separated pad and access road gravel to finish top 6” of excavation and compact. Or provide 6” new “like” materials to finish pad and access road surface.

CSI 26 - Electrical

1. Specification covers requirements for electrical safety and security when conducting with/around/by electrical operations. All installation and connections will be done in accordance with the National Electrical Codes (NEC).

2. Conduits

a. Remove wires from existing conduit to be replaced.

i. LSB to power pole at all five (5) sites (approximately 103 Lineal feet each) and LSB to gate switch at H-09 and I-04 (approximately 70 Lineal feet each).

ii. Terminate and cap each end of all existing conduits a minimum of 12” below grade and abandon in place.

b. Place new 1 ½” PVC coated rigid conduit, properly terminate conduit at the LSB and power pole light and/or gate switch as required.

c. Pull new 10/3 copper stranded wire from LSB to power pole, utilize pull box at pole and gate switch as required.

d. Properly terminate wire at site light or gate switch as required to provide fully functional site lighting.

e. Gate Switches – Replace switches with Leviton 1221-21 Single-Pole 20A 277Volt Ivory Ac

Switch. Save existing switch and return to 90 CEM.

CSI 31 – Earthwork – Excavation and Fill

1. Specification covers requirements for furnishing all equipment, materials, labor, tools, and techniques for earthwork conducted pre/during/post construction.

2. The Contractor shall comply with AF requirements as identified in the section labelled, “Excavation,” prior to commencement of work.

a. All excavation work shall be performed to neat lines to the depths indicated in this SOW and

Drawings.

b. Where required, hand excavation techniques shall be utilized to avoid damage to existing underground utilities or any other services.

c. The top six (6) inches of materials from the pad and/or access road will be separated and stored for use as final fill lift of the conduit trench. The contractor will be responsible for providing a like gravel material to meet the final 6” lift on the pad and access road.

d. Conduit trench will be a minimum of 36 inches deep with no sharp rocks or other deleterious materials exposed to the conduit.

e. Material shall be removed so as not to impede subsequent material placement, grading, and/or compaction operations.

f. Material that is deemed incompatible with specified new materials shall be become the property of the Contractor.

i. No material deemed as waste is to be disposed of on AF property.

ii. Appropriate removal and disposal is required to be completed by the Contractor in accordance with local, state, and federal requirements.

iii. The Contractor shall be responsible for all material removed from the AF property and the Contractor shall make all necessary arrangements for the proper disposal of the material to include payment of any applicable fees or permits.

iv. The waste of suitable backfill material, removed as part of the excavation, shall not be permitted where conditions exist for this material’s reuse in the construction activities at the site.

v. The Contractor is responsible for supplying equivalent imported fill material to offset any wasted suitable fill at no cost to the Government.

vi. The top 6” of the fill crossing the pad or access road will be the original, reclaimed gravel materials or new like gravel materials.

3. Backfill

a. Unsuitable excavated soils shall not be used as fill material.

b. Unsuitable excavated materials shall be disposed of by the Contractor in accordance with this

SOW.

c. Excavated soils which meet the requirements of the fill material as described in this SOW may be re-used in accordance with this SOW.

d. Suitable backfill materials shall be clean, well graded, and free of fines, clays, organics, frozen or any other deleterious materials.

e. Backfill material shall be placed in six-inch (6”) lifts, mechanically compacted in accordance with this SOW.

f. Backfill material shall be placed to maintain the existing ground surface, drainage flows, and elevations shown on the Drawings.

g. Compactive efforts shall be sufficient to meet the equivalent testing requirements for maximum density as determined by the following criteria unless otherwise noted in the Bid Drawings.

h. Subgrade preparation and fill in unpaved areas – 95% ASTM D698 or 90% ASTM D1557 within +2% and -4% of optimum moisture content.

i. Fill and imported base material in paved areas – 100% ASTM D698 or 95% ASTM D1557 within +1% and -2% of optimum moisture content

j. Compactive effort may be evaluated by laboratory testing in accordance with this SOW at the discretion of 90 CES/CEM.

k. Any material which fails the laboratory tests shall be re-compacted or removed in its entirety, replaced, re-compacted, and re-tested at no cost to the Government until the material successfully passes the laboratory tests.
