About this file

This is a Department of Interior Past Performance Questionnaire (PPQ) template used to collect references and evaluations of contractor performance on previous projects. The questionnaire is designed as a two-part process where offerors initially complete Section A with project and company information, then submit the form to project owners or evaluators who complete Section B with their assessment. Completed questionnaires must be received by the contracting point of contact by the deadline specified in the solicitation, with late submissions not accepted. Each offeror may submit only one past performance record per project, and all referenced projects must have been completed within five years preceding the solicitation date unless otherwise specified.

Section B of the questionnaire requires evaluators to rate contractor performance across five assessment areas using a six-point scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable). The evaluation areas include: Quality (workmanship, quality control, and technical submittals); Schedule and Time Management (milestone adherence, responsiveness, and timely submissions); Pricing and Payments (cost management, modification pricing, invoicing accuracy, and subcontractor payments); Management (oversight, subcontractor coordination, safety, remote location capability, owner relations, and problem-solving); and Regulatory Compliance (building codes, life safety codes, and labor standards). Evaluators must provide supporting comments for all ratings, with particular emphasis on explaining exceptional, marginal, and unsatisfactory ratings. The form concludes with an overall recommendation regarding the contractor's suitability for similar future contracts and requires evaluator signature from the client or owner.
