About this file
This document is a Construction Bid Submission Review Form created by the State of West Virginia Department of Environmental Protection (DEP) on August 11, 2017. The form serves as an informational guide for potential bidders on construction projects and outlines common procedural errors that may result in bid disqualification. The document emphasizes that bidders must thoroughly review all request for quotation documents, additional materials, and instructions in their entirety, as the form does not represent a comprehensive list of all bidding requirements. The form identifies ten errors that constitute immediate grounds for bid disqualification, including failure to attend mandatory pre-bid meetings, failure to sign required signature pages, failure to provide valid bid bonds, non-compliance with mandatory RFQ requirements, failure to acknowledge receipt of addenda when required, late bid submission, federal debarment status, West Virginia state debarment or suspension, outstanding environmental enforcement actions, and unresolved DEP vendor complaints.
The document also identifies five additional categories of errors that may result in bid disqualification before contract award, requiring resolution prior to award. These include outstanding debt to the state or political subdivisions, workers' compensation or unemployment compensation delinquency, failure to obtain required bonds and insurance, failure to supply a West Virginia contractor's license number within one day of DEP request, failure to submit a signed drug-free workplace affidavit within one day of DEP request, and failure to provide a subcontractor listing within one business day of bid opening or DEP request. The form functions as a procedural checklist rather than a pricing or contract terms document and does not address specific project scope, funding sources, or award timelines.
