About this file

This is a Construction Bid Submission Review Form created by the State of West Virginia Department of Environmental Protection (DEP) on August 11, 2017. The document serves as an informational guide for potential bidders on construction projects, outlining common bidding requirements and potential reasons for bid disqualification. The form emphasizes that bidders must thoroughly review all request for quotation documents, including the RFQ, supporting documents, and related instructions, to identify specific project requirements. The document identifies ten critical errors that result in immediate bid disqualification, including failure to attend mandatory pre-bid meetings, failure to sign required signature pages, failure to supply valid bid bonds, non-compliance with mandatory RFQ requirements, failure to acknowledge addenda when required, late bid submission, federal or state debarment or suspension, outstanding environmental enforcement actions, and unresolved DEP vendor complaints.

The form also identifies five categories of errors that may result in bid disqualification before contract award. These include unresolved debt to the state or political subdivision, workers' compensation or unemployment compensation delinquency, failure to obtain required bonds and insurance, failure to supply a West Virginia contractor's license number within one day of request, failure to supply a signed drug-free workplace affidavit within one day of request, and failure to provide a subcontractor listing within one business day of bid opening or DEP request. The document explicitly states it is not a comprehensive list of all bidding requirements and does not guarantee coverage of every potential oversight that could result in disqualification, emphasizing that bidders bear responsibility for full compliance with all stated bid document requirements.
