About this file

This is a Statement of Principal Place of Business form required by the School Board of Polk County, Florida for bidders responding to an Request for Proposals (RFP) for Equitable Services for Private Schools. The form must be completed by all bidders and returned with their bid responses. Bidders with principal places of business located within Florida need only provide basic identifying information and submit the completed form. The procurement seeks proposals from qualified firms to deliver instructional programs including tutoring in reading, mathematics, science, and academic counseling; professional development for private school teachers; and parent involvement activities for eligible Title I participants in private schools. Respondents must propose all three service components to be considered responsive to the RFP.

For bidders whose principal place of business is located outside Florida, completion of the form requires submission of a written legal opinion from an attorney licensed to practice law in the bidder's home state. This opinion must address whether the bidder's state or any political subdivision thereof grants bidding preferences to in-state business entities in public contracts, in accordance with Florida Statutes Section 287.084(2). The attorney must identify the state or political subdivision, describe any applicable preferences, cite relevant laws or authorities, and provide complete contact information including signature, printed name, address, telephone number, email address, and states of bar admission. Failure to comply with these requirements renders the bid non-responsive to the solicitation terms.
