About this file

This document is an Offer and Acceptance form from Yavapai College Procurement & Contract Services located in Prescott, Arizona, designed to establish terms for potential goods or services procurement. The form provides a template for vendors to submit offers, with spaces for initial and best and final offers, and includes comprehensive certification requirements for potential contractors. Vendors must commit to non-discrimination, avoid conflicts of interest, comply with state and federal regulations, and submit a firm offer valid for 180 days following the solicitation due date.

The document outlines specific legal and ethical certifications that offerors must meet, including prohibitions against collusion, economic inducements to public servants, and involvement in anti-competitive practices. Importantly, contractors are cautioned not to commence any billable work until receiving an official purchase order, contract release document, or written notice to proceed from an authorized Yavapai College employee. The form was updated on 07/01/24 and provides a structured approach for potential contractors to formally propose services while affirming compliance with various state and federal regulatory requirements.
