About this file
This addendum document supplements the Request for Proposals issued by the State of Florida, Agency for Health Care Administration (AHCA), for the development and operation of a web-based organ and tissue donor registry coupled with a statewide public education initiative. The addendum, posted on July 23, 2026, provides responses to written questions submitted by prospective vendors and modifies specific requirements of the original RFP 015-25/26. Proposals are due August 10, 2026 at 2:00 p.m., with written questions required by July 7, 2026 at 2:00 p.m. The contract term spans five years from October 1, 2026 through September 30, 2031, with an additional three-year renewal option available subject to performance evaluation and fund availability. Implementation must be completed within three months of contract execution, and the anticipated Notice of Intent to Award is September 8, 2026.
The estimated contract value is $702,000 over the five-year period, consisting of $342,000 in implementation costs and $72,000 in annual operations and maintenance costs. A proposal guarantee of $70,200 and a 10 percent annual performance bond are required from selected vendors. Priority consideration is given to existing nonprofit organizations based in Florida with demonstrated expertise in organ and tissue donation, procurement organization partnerships, and statewide donor education campaigns. Funding derives from the Health Care Trust Fund, though contractors may also receive and utilize voluntary contributions. Respondents must submit comprehensive proposals addressing twelve submission requirement components and provide two verifiable non-agency client references for similar work, along with certifications regarding conflicts of interest, non-collusion, drug-free workplace compliance, E-Verify verification, and scrutinized company list compliance.

A Submission Requirements and Evaluation Criteria Components
Instructions to Respondents for the Completion of Exhibit A
A-4 2 Responses must be made directly into the SRC form (pg.2), but the text field does not allow for the selection of font, font size, or formatting of the page size (pg 1). Those requirements seem to contradict each other. How can font, font size and page size be maintained when providing responses directly into the PDF?
