This is Addendum No. 3 Final to Request for Proposal RFP 26-025-CJ issued by the County of Gloucester Central Purchasing Department for Gloucester County Public Schools (GCPS) Employee Medical, Stop Loss, Dental, Vision, and Voluntary Benefits Coverage. The RFP seeks comprehensive insurance coverage proposals including medical, stop-loss, dental, vision, hearing aid rider, and employee assistance program (EAP) benefits. The proposal submission requires one original hard copy with wet signatures and one electronic version on USB, with a 14-page limit for single-sided pages (excluding specified attachments such as the Proposal Form, Price Proposal in Excel format, references, W-9, and required analytical files). The proposal due date is April 1, 2026, at 4:30 PM. Finalist interviews are anticipated for May 26-29, 2026, with Board approval scheduled for July 14, 2026, and award expected following Board approval. The stop-loss contract period is paid in 12 months.
Key specifications include quotations for four vision tiers (employee only, employee plus one, employee plus children, and employee plus family) using an open pharmacy formulary, and wellness funding allocation of $30,000 annually with no carryover. The current medical plan options include Anthem HK POS and Anthem HK HSA plans with specified employee census data provided, including retiree enrollment by plan type. Claims data, high-cost claims reports, and premium equivalents are provided for underwriting purposes, covering multiple plan years from October 2023 through current date. Electronic signatures are not acceptable; all submissions require original wet signatures on the Proposal Form. Certain attachments, including pharmacy questionnaires, implementation plans, and pharmacy pricing, must be submitted electronically only. The electronic password for accessing the Medical Claims Repricing File is "Celineglo." A signed acknowledgment of this addendum should be attached to offers but will not disqualify submissions if absent.

Question: Please confirm whether electronic signatures are acceptable.
Response: Electronic signatures are not acceptable. The RFP requires that the Proposal Form be signed with original signatures, not photocopies, indicating a wet signature is required for the hard copy submission.
Question: Can exhibits (such as signed addenda, sample contracts, flyers, etc.) be included only in the electronic submission, or do they also need to be printed in the hard copy version?
Response: All required components of the proposal should be included in the printed hard copy submission unless specifically identified as electronic-only. Certain analytical files (e.g., Excel pricing and claims data files) are to be submitted electronically and do not need to be printed; however, supporting exhibits that are part of the proposal should be included in the hard copy unless otherwise noted.
Question: For the retirees included in the census, are you able to provide which plan option each retiree has selected? The current census does not specify the plan enrollment.
Response:

Response: There are 4 Tiers- EE only, EE + one, EE + Children, EE +Family
Question: Do we know when the amendment to address questions will be released? Since a printed copy is needed, we need to finalize our internal submission the week before the due date to ensure timely delivery.
Response: Addenda will be issued by the Purchasing Agent and posted on the County’s website and eVA.
While a specific release date is not provided, addenda are typically issued after the question deadline (March 18, 2026) and prior to the proposal due date. Offerors are responsible for monitoring for all addenda.
