About this file
This addendum modifies the evaluation criteria for the New Hampshire Department of Health and Human Services' Request for Proposals RFP-2026-DMS-04-COMMU for Community Reentry Navigator and Communication Services. The original solicitation, published February 26, 2026, requests proposals from qualified vendors to provide navigator services supporting correctional facilities and incarcerated individuals across all fifteen New Hampshire carceral facilities. Services include assisting with Medicaid enrollment under the 1115 demonstration benefit and youth reentry benefit for prelease services, applications for other health insurance coverage, preparation for community reentry, transition support out of the Medicaid program, and post-release community engagement requirement compliance for individuals remaining Medicaid-eligible. Additionally, the selected vendor must develop and disseminate educational materials to inform incarcerated individuals and the general public about the community reentry program. Responses are due by March 26, 2026, with the contract term covering three fiscal periods: through June 30, 2026, July 1, 2026 through June 30, 2027, and July 1, 2027 through December 31, 2027.
Addendum #1 modifies Section 3.1 of the solicitation to establish the evaluation scoring structure as follows: Technical Response comprises 1,150 possible points, allocated as 430 points for Experience (Q1), 430 points for Ability (Q2), and 290 points for Capacity (Q3); Cost Proposal comprises 450 possible points based on a formula; the maximum possible score is 1,600 points. The Department has clarified that this is not a low-cost award, indicating that technical qualifications will carry substantial weight in vendor selection. The addendum does not disclose the overall contract award value, maximum price limitation, set-asides, or incumbent vendor information in the available portion of the document.
