About this file

This document is an Addendum (#01) issued by the Polk County School Board's Purchasing Department for Bid Number 059-MHH-0615, covering Septic Tank, Lift Stations, and Grease Trap Service - Term Contract. The addendum, dated June 15, 2015, provides clarifications and responses to vendor questions for a one-year contract with renewal options. The bid requires bidders to submit a vehicle registration form for residuals transporter/applicator, demonstrating equipment availability, including at least two pump trucks with one having a minimum 4,000-gallon capacity. The contract allows for potential separate awards for different service categories (lift stations, septic tanks, and grease traps).

Key bid requirements include submitting references and acknowledging the addendum, with written questions having been accepted until 4:00 pm on June 11, 2015. The addendum addresses specific vendor inquiries, confirming that references are required, grease traps may be awarded separately from other services, and a minimum of two metered trucks are necessary for the lift station services. The contract includes an option to renew, as specified in the General Instructions and Conditions, providing flexibility for the Polk County School District in maintaining these essential maintenance services.

Q1. Since we have worked with you in the past, would you like us to submit references as it says we might or might not need to?

A1. Yes, all bidders should submit references as indicated in section P of the Special Terms and Conditions on pages 8, 9 and 10 in Invitation to Bid file #

059-MHH-0615.

Q2. I believe I read that the grease traps may be awarded separately from the lift stations and septic tanks? Is this correct??

A2. Yes, please see section K.2 in the Special Terms and Conditions on page 7 of 21 and Section P.1. in the Special Terms and Conditions on page 9 in Invitation to Bid file # 059-MHH-0615.

A3. Yes, please see the change to Section P.3. at the top of page 2 of ADDENDUM #01 to bid file # 059-MHH-0615.

Q4. I noticed this contract will be for 1 year, are there any renewals for the awarded contractor or will it go out to bid again next year?

A4. Yes there is an option to renew. Please see section 21 on page 3 in the General Instructions and Conditions of Invitation to Bid file # 059-MHH-0615.
