About this file

This is an Infection Prevention & Control Construction Risk Assessment (ICRA) form for a federal healthcare facility project.

The document establishes a three-step assessment framework for the project "Abate Central Chase Replace Fire Main 667-18-111" at Overton Brooks VAMC, dated September 25, 2025. Step One classifies construction projects into four types: Type A (inspection and non-invasive activities), Type B (small-scale, short-duration activities creating minimal dust, including the project-specific activity of cutting through plaster walls in hallways), Type C (moderate to high dust generation or demolition/removal activities), and Type D (major demolition and construction requiring consecutive work shifts). Step Two identifies affected patient risk groups across four categories: Low Risk (office areas, public access areas), Medium Risk (clinical departments including cardiology, endoscopy, radiology), High Risk (emergency room, surgical units, pharmacy, sterile supply storage), and Highest Risk (immunocompromised patient care areas, operating rooms, intensive care units). Step Three applies an Infection Control Matrix that cross-references construction type with patient risk group to determine required precaution class levels (I through IV). The assessment was completed and signed by Carol Phillips, RN, CIC (Certified Infection Preventionist). This ICRA document serves as the infection control planning tool to establish protective measures based on construction scope and patient population proximity.

Overton Brooks VAMC Infection Prevention & Control Construction Risk Assessment (ICRA)

Project: Abate Central Chase Replace Fire Main 667-18-111 Date: 09/25/2025 STEP ONE: CIRCLE the Type of Construction Project based on scope and expected activity (Type A-D)

TYPE

A

Inspection and Non-Invasive Activities. Includes, but is not limited to:

removal of ceiling tiles for visual inspection only, e.g., limited to 1 tile per 50 square feet painting (but not sanding) wallcovering, electrical trim work, minor plumbing, activities which do not generate dust or require cutting of walls or access to ceilings other than for visual inspection.

TYPE

B

Small scale, short duration activities which create minimal dust Includes, but is not limited to:

installation of telephone and computer cabling access to chase spaces cutting of walls or ceiling where dust migration can be controlled.

Project Specific: cutting through plaster walls in hallway required

TYPE

C

Work generates a moderate/high level of dust or requires demolition/removal of any fixed building components or assemblies Includes, but is not limited to:

sanding of walls for painting or wall covering removal of floorcoverings, ceiling tiles and casework; new wall construction minor duct work or electrical work above ceilings major cabling activities any activity which cannot be completed within a single work shift.

TYPE

D

Major demolition and construction projects Includes, but is not limited to:

activities which require consecutive work shifts requires heavy demolition, removal of a complete cabling system, or excavation new construction.

STEP TWO: CIRCLE the affected Patient Risk Groups. If more than one risk group will be affected, select the higher risk group.
