About this file
This is an Infection Prevention & Control Construction Risk Assessment (ICRA) form for the Overton Brooks Veterans Affairs Medical Center (VAMC) for the project "Abate Central Chase Replace Fire Main 667-18-111," dated September 25, 2025, and signed by Carol Phillips, Infection Prevention Officer.
The assessment determines infection control precautions required for construction activities based on two primary factors: construction project type and affected patient risk groups. The project has been classified as TYPE B (small scale, short duration activities creating minimal dust), specifically noting that cutting through plaster walls in hallway is required. The affected patient risk group is identified as HIGH Risk, which includes areas such as Emergency Rooms, Laboratories, Medical Units, Outpatient Surgery, Pharmacy, Surgical Units, and Sterile Supply storage/distribution. Using the IC Matrix that correlates construction type with patient risk, the assessment determines that Class II/II infection control precautions are required. This classification indicates the need for moderate infection control measures appropriate for a small-scale construction project in a high-risk patient care area, ensuring that dust migration can be controlled and patient safety is maintained during the fire main replacement work in the central chase space.

Infection Prevention & Control Construction Risk Assessment (ICRA)
Project: Abate Central Chase Replace Fire Main 667-18-111 Date: 09/25/2025
STEP ONE: CIRCLE the Type of Construction Project based on scope and expected activity (Type A-D)
TYPE
A
Inspection and Non-Invasive Activities. Includes, but is not limited to:
▪ removal of ceiling tiles for visual inspection only, e.g., limited to 1 tile per 50 square feet
▪ painting (but not sanding)
▪ wallcovering, electrical trim work, minor plumbing, activities which do not generate dust or require cutting of walls or access to ceilings other than for visual inspection.
TYPE
B
Small scale, short duration activities which create minimal dust Includes, but is not limited to:
▪ installation of telephone and computer cabling
▪ access to chase spaces
▪ cutting of walls or ceiling where dust migration can be controlled.
▪ Project Specific: cutting through plaster walls in hallway required
TYPE
C
Work generates a moderate/high level of dust or requires demolition/removal of any fixed building components or assemblies Includes, but is not limited to:
▪ sanding of walls for painting or wall covering
▪ removal of floorcoverings, ceiling tiles and casework; new wall construction
▪ minor duct work or electrical work above ceilings
▪ major cabling activities
▪ any activity which cannot be completed within a single work shift.
TYPE
D
Major demolition and construction projects Includes, but is not limited to:
▪ activities which require consecutive work shifts
▪ requires heavy demolition, removal of a complete cabling system, or excavation
▪ new construction.
STEP TWO: CIRCLE the affected Patient Risk Groups. If more than one risk group will be affected, select the higher risk group.
