About this file
This is a Statement of Special Inspections for a structural project at Camp Nelson National Monument Fee Memorial Church in Nicholasville, Kentucky, under the ownership of the National Park Service. Prepared by Phillip M. Holthaus, P.E., S.E., the document establishes the special inspection and structural testing requirements required as a condition for permit issuance. The Statement encompasses structural discipline inspections and identifies building systems subject to inspection including soils and foundations, cast-in-place concrete, cast-in-place deep foundations, and light frame wood walls with structural wood shear panels. The document designates a Seismic Design Category B classification and requires a Quality Assurance Plan for seismic resistance. All inspectors and testing agencies must be engaged by the Owner or Owner's Agent rather than the Contractor, with credentials subject to Building Official approval. Interim reports shall be submitted weekly or per an attached schedule, with final reports documenting completion of all required inspections and testing required prior to Certificate of Use and Occupancy issuance.
The document specifies detailed inspection and testing scopes across multiple disciplines. Soils inspections require PE/GE qualification and include verification of bearing capacity, excavation depth, and controlled fill material classification and compaction. Cast-in-Place Concrete inspections with ACI-CCI or ICC-RCSI qualifications cover mix design compliance, material certification, reinforcement installation, curing protection, anchor rods, formwork, concrete placement, and concrete testing including compressive strength per ASTM standards with sampling at 7 day and 28 day intervals. Cast-In-Place Deep Foundations require PE/GE oversight of drilling operations, placement verification, and plumbness confirmation. The document provides a comprehensive qualifications matrix detailing required certifications from organizations including American Concrete Institute (ACI), American Welding Society (AWS), International Code Council (ICC), and NICET across multiple specialty inspection categories. The Special Inspection Coordinator maintains all records and furnishes reports to the Building Official and Design Professional in Responsible Charge, with discovered discrepancies escalated for correction.

Item Agency #
(Qualif.)
Scope
1. Soils PE/GE Periodically verify that soils below footings for adequate bearing capacity and consistency with geotechnical report.
Periodically verify that excavations are extended to proper depth and have reached proper material.
Periodically perform classification and testing of controlled fill materials.
Continuously verify the use of proper materials, densities and lift thicknesses during placement and compaction of controlled fill.
Prior to placement of controlled fill, periodically observe the subgrade and verify that the site has been prepared properly.
